Estrogen Plus Progestin Therapy

Cardiovascular Disorders and Probable Dementia

The Women's Health Initiative (WHI) estrogen plus progestin substudy reported increased risks of pulmonary embolism (PE), deep vein thrombosis (DVT), stroke, and myocardial infarction (MI) in postmenopausal women (50 to 79 years of age) during 5.6 years of treatment with daily oral conjugated estrogens (CE) [0.625 mg] combined with medroxyprogesterone acetate (MPA) [2.5 mg], relative to placebo.

The WHI Memory Study (WHIMS) estrogen plus progestin ancillary study of WHI reported an increased risk of developing probable dementia in postmenopausal women 65 years of age and older during 4 years of treatment with daily CE (0.625 mg) combined with MPA (2.5 mg), relative to placebo. It is unknown whether this finding applies to younger postmenopausal women.

Do not use estrogen plus progestogen therapy for the prevention of cardiovascular disease or dementia.

Breast Cancer

Prescribe estrogens with or without progestogens at the lowest effective doses and for the shortest duration consistent with treatment goals and risks for the individual woman.

About Beyaz

Overview

Beyaz is a prescription contraceptive tablet containing drospirenone, ethinylestradiol and levomefolate calcium, used to prevent pregnancy. It may also be prescribed for moderate acne or premenstrual dysphoric disorder (PMDD) in women who choose an oral contraceptive for birth control. Drospirenone and ethinylestradiol work mainly by preventing the ovaries from releasing an egg. Levomefolate calcium supplies folate to help maintain the body’s folate levels.

What it treats

- Prevention of pregnancy. - Premenstrual dysphoric disorder (PMDD) in women who also choose oral contraception. - Moderate acne in females aged 14 years and older who have started menstruating and also want oral contraception. - Folate supplementation to help reduce the risk of neural tube defects if pregnancy occurs during use or shortly after stopping.

Safety information

Side effects

Common side effects include nausea, headache, breast tenderness, spotting or bleeding between periods, and mood changes. Serious side effects can include blood clots, stroke, heart attack, high blood potassium, liver problems and severe allergic reactions. Seek emergency medical help for: • Sudden chest pain, shortness of breath or coughing up blood. • Pain or swelling in one leg. • A sudden severe headache, sudden vision changes, difficulty speaking, or weakness or numbness on one side of the body. • Swelling of the face, lips, tongue or throat, or difficulty breathing. Contact a doctor promptly for an irregular heartbeat, marked muscle weakness, severe or persistent abdominal pain, yellowing of the skin or eyes, or new or worsening depression. Drospirenone can raise potassium levels, which may affect heart rhythm.

When to call your doctor

- Pain or swelling in one leg, especially with warmth or redness. - Sudden chest pain, trouble breathing or coughing up blood. - A sudden severe headache, sudden vision changes, trouble speaking, or weakness or numbness on one side of the body. - Severe abdominal pain or yellowing of the skin or eyes. - Swelling of the face, lips, tongue or throat, or difficulty swallowing or breathing.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not take Beyaz if you have: • A current or previous blood clot in the legs or lungs, stroke, heart attack, or a condition that puts you at high risk of blood clots. • Migraine with aura, or any migraine if you are over 35. • Uncontrolled high blood pressure, diabetes with blood vessel damage, or certain heart valve or heart rhythm disorders. • Kidney impairment, adrenal insufficiency, liver disease or liver tumours. • Current or previous breast cancer or another cancer sensitive to female hormones. • Unexplained vaginal bleeding. • Known or suspected pregnancy. • An allergy to drospirenone, ethinylestradiol, levomefolate calcium or any tablet ingredient. Beyaz should not be used by women over 35 who smoke, because smoking increases the risk of serious cardiovascular problems. Certain hepatitis C treatment combinations must not be taken with Beyaz because they can increase liver enzymes; the prescriber must check compatibility. Medical review is needed before use if you have high blood pressure, diabetes, high blood fats, gallbladder disease, depression, or a family history of blood clots. Drospirenone can raise blood potassium, so medicines or supplements that also raise potassium require review and may require blood tests. Planned major surgery, prolonged immobility, recent childbirth and breastfeeding also require assessment; Beyaz may be unsuitable or may need to be stopped temporarily.

Interactions

Tell your doctor about all medicines, supplements and herbal products you take. Some medicines can make Beyaz less effective at preventing pregnancy or cause unexpected bleeding. These include rifampicin, certain seizure medicines such as carbamazepine, phenytoin, oxcarbazepine and topiramate, and St John’s wort. Some HIV and hepatitis C treatments also interact. Additional contraceptive precautions may be needed. Drospirenone can raise potassium levels. Your doctor should know if you take potassium supplements, potassium-containing salt substitutes, ACE inhibitors, angiotensin receptor blockers, potassium-sparing diuretics such as spironolactone, heparin, or regular long-term NSAID painkillers such as ibuprofen or naproxen. Blood potassium checks may be needed. Some antifungal medicines, including ketoconazole and itraconazole, and antibiotics such as clarithromycin can increase hormone levels and the risk of side effects. Grapefruit juice may also increase hormone levels; discuss its use with your doctor. Beyaz can lower lamotrigine levels, which may affect seizure control. The levomefolate component can also interact with certain seizure medicines and medicines that affect folate, such as methotrexate. Certain hepatitis C combinations, including ombitasvir/paritaprevir/ritonavir with or without dasabuvir, must not be used with Beyaz because they can increase liver enzymes. Have your doctor check compatibility before starting a new treatment.

Dosing

Dosing is set by your prescriber. Each health care professional may prescribe medication differently according to a patient's medical history and condition, so follow the directions on your prescription label and speak to your doctor or pharmacist before changing anything.

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
