When to call your doctor
If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
Do not take this medicine if you have a known allergy to diclofenac or any of its ingredients, including a previous severe allergic reaction or serious skin reaction. Do not take it if aspirin or other nonsteroidal anti-inflammatory drugs (NSAIDs) have caused asthma, hives or other allergic reactions. Severe, sometimes fatal allergic reactions to NSAIDs have occurred in these patients. This medicine must not be used in connection with coronary artery bypass graft (CABG) surgery.
Interactions
Tell your doctor about other medicines you take, particularly: Blood thinners such as warfarin, antiplatelet medicines such as aspirin, and SSRI or SNRI antidepressants: taking these with diclofenac increases the risk of bleeding. Your doctor should monitor for signs of bleeding. Aspirin used for pain, other NSAIDs, and salicylates such as diflunisal or salsalate: combining these with diclofenac is not recommended because it increases stomach and bowel side effects, including bleeding, with little or no added benefit. Diclofenac does not replace aspirin prescribed for heart protection. Blood pressure medicines, including ACE inhibitors, angiotensin receptor blockers (ARBs), and beta-blockers, and diuretics (“water tablets”): diclofenac may make these medicines less effective. Combining it with ACE inhibitors or ARBs can worsen kidney function, particularly in older people, people who are dehydrated, or those with kidney problems. Blood pressure and kidney function may need monitoring. Digoxin and lithium: diclofenac can increase their levels in the blood. Your doctor should monitor digoxin levels and watch for signs of lithium toxicity. Methotrexate and cyclosporine: diclofenac may increase methotrexate toxicity, including low blood cell counts and kidney problems, and may increase kidney harm from cyclosporine. Monitoring is needed. Pemetrexed: diclofenac may increase its harmful effects on blood cell production, the kidneys, and the stomach or bowel. Diclofenac should be avoided around pemetrexed treatment; your treating doctor should manage the timing. Voriconazole and rifampin: these affect an enzyme involved in breaking down diclofenac. Voriconazole may increase diclofenac exposure and toxicity, while rifampin may reduce its effectiveness. Your prescriber may need to adjust treatment.
