About Celexa

Overview

Celexa is a prescription medicine containing citalopram. It is used to treat major depressive disorder in adults. Citalopram is a selective serotonin reuptake inhibitor (SSRI), which means it works by reducing the reabsorption of serotonin.

What it treats

Major depressive disorder (MDD) in adults.

Side effects

The most common side effect listed is ejaculation problems, mainly delayed ejaculation. Citalopram may also cause other sexual problems. Serious side effects that warrant contacting a doctor include: • Suicidal thoughts or behaviours in adolescents and young adults. • Changes in the heart’s electrical rhythm, including dangerous irregular heartbeats that can cause sudden death. • Serotonin syndrome, which can occur with citalopram alone or when taken with other medicines that affect serotonin. • Increased bleeding risk, especially when taken with aspirin, nonsteroidal anti-inflammatory drugs, other antiplatelet medicines or anticoagulants such as warfarin. • Allergic reactions. • Mania or hypomania, involving unusually elevated mood or activity. • Seizures. • Angle-closure glaucoma, a serious eye condition. • Low blood sodium. • Discontinuation symptoms when stopping treatment.

When to call your doctor

• Thoughts of suicide or harming yourself, or worsening mental health. • A new fast or irregular heartbeat. • A seizure. • Unusual bleeding. • An unusually elevated mood or extreme overactivity, which may be signs of mania.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not take Celexa (citalopram) if you: • Take a monoamine oxidase inhibitor (MAOI), or have stopped one within the past 14 days. This includes linezolid and intravenous methylene blue. The combination increases the risk of serotonin syndrome. • Take pimozide, because of the risk of abnormal heart rhythms. • Are allergic to citalopram or any ingredient in the tablets. Serious allergic reactions, including anaphylaxis and swelling, have been reported. Citalopram should generally be avoided if you have congenital long QT syndrome, a slow heart rate, low potassium or magnesium, a recent heart attack, or heart failure that is not adequately controlled, or if you take other medicines that prolong the QT interval. It can cause serious, potentially fatal heart rhythm disturbances. Electrolyte monitoring may be needed in people at risk of low potassium or magnesium. It should also be avoided in people with untreated narrow drainage angles in the eyes, because of the risk of angle-closure glaucoma. Tell your prescriber if you have a seizure disorder or bipolar disorder. Screening for bipolar disorder is needed because citalopram may trigger mania or hypomania. Other medicines that increase serotonin can raise the risk of serotonin syndrome, although this can also occur with citalopram alone. Aspirin, nonsteroidal anti-inflammatory drugs, antiplatelet medicines and anticoagulants such as warfarin may increase bleeding risk when used with citalopram. Low blood sodium and sexual dysfunction can also occur. If you are pregnant, discuss treatment with your prescriber. SSRI use, particularly late in pregnancy, may increase the newborn’s risk of persistent high blood pressure in the lungs and symptoms such as breathing or feeding difficulties, unstable temperature, low muscle tone, tremor and irritability.

Interactions

Do not take citalopram with monoamine oxidase inhibitors (MAOIs), including linezolid or intravenous methylene blue, because of the increased risk of serotonin syndrome. Citalopram must not be taken with pimozide because it can increase pimozide levels and the risk of serious heart rhythm problems. Other medicines that prolong the QT interval, a change in the heart’s electrical activity, should be avoided with citalopram. Medicines that inhibit the enzyme CYP2C19 also increase the risk of heart rhythm problems; your prescriber must account for this when setting your dose. Tell your doctor if you take other medicines or supplements that affect serotonin, including other SSRIs, SNRIs, triptans, tricyclic antidepressants, opioids, lithium, buspirone, amphetamines, tryptophan or St. John’s wort. These combinations increase the risk of serotonin syndrome and require monitoring, especially when starting citalopram or after a dose increase. Antiplatelet medicines and anticoagulants can increase bleeding risk when taken with citalopram. If you take warfarin, your INR blood test results need careful monitoring.
