Suicidality and Antidepressant Drugs
Antidepressants increased the risk compared to placebo of suicidal thinking and behavior (suicidality) in children, adolescents, and young adults in short-term studies of major depressive disorder (MDD) and other psychiatric disorders. Anyone considering the use of amitriptyline hydrochloride tablets or any other antidepressant in a child, adolescent, or young adult must balance this risk with the clinical need. Short-term studies did not show an increase in the risk of suicidality with antidepressants compared to placebo in adults beyond age 24; there was a reduction in risk with antidepressants compared to placebo in adults aged 65 and older. Depression and certain other psychiatric disorders are themselves associated with increases in the risk of suicide. Patients of all ages who are started on antidepressant therapy should be monitored appropriately and observed closely for clinical worsening, suicidality, or unusual changes in behavior. Families and caregivers should be advised of the need for close observation and communication with the prescriber. Amitriptyline hydrochloride tablets are not approved for use in pediatric patients (see Warnings: Clinical Worsening and Suicide Risk, Precautions: Information for Patients, and Precautions: Pediatric Use.)
About Elavil
Overview
Elavil is a prescription medicine containing amitriptyline hydrochloride. It is used to relieve symptoms of depression. It comes as tablets.

Safety information
Side effects
Possible side effects include dry mouth, constipation, nausea, vomiting, diarrhoea, changes in taste, drowsiness, dizziness, headache, weakness and tiredness. Blurred vision, sweating, changes in appetite or weight, sleep problems, anxiety, restlessness and changes in sexual function can also occur. The label does not state how often these effects occur. Serious effects that warrant prompt medical attention include heart attack, stroke, irregular heartbeat, heart block, fainting, seizures, hallucinations, severe confusion and abnormal involuntary movements. Other serious effects include inability to pass urine, stopped bowel movement due to intestinal paralysis, increased pressure in the eye, very high fever, facial or tongue swelling, and blood-cell disorders. Liver inflammation, sometimes with jaundice, has rarely been reported. Very rare reports include heart muscle disease and serotonin syndrome when amitriptyline is taken with other medicines associated with that syndrome. A very rare reaction resembling neuroleptic malignant syndrome has also been reported after starting treatment or increasing the prescribed dose. Symptoms include muscle stiffness, fever, changes in mental state, sweating, a fast heartbeat and tremor. Stopping abruptly after prolonged treatment may cause nausea, headache and feeling unwell. Irritability, restlessness and disturbed dreams or sleep have also been reported during gradual withdrawal. These symptoms do not indicate addiction. Mania or hypomania has rarely occurred after stopping long-term treatment. The label includes some reactions reported with related tricyclic antidepressants, rather than specifically with amitriptyline. Liver failure, loss of taste and a lupus-like syndrome have also been reported, but a causal link has not been established.

Before taking this medicine
Do not take Elavil (amitriptyline) if you have previously had an allergic reaction to amitriptyline. Do not take it with a monoamine oxidase inhibitor (MAOI) or within 14 days after stopping an MAOI. Combining these medicines can cause dangerously high fever, severe seizures and death. Do not take Elavil with cisapride because the combination can prolong the heart’s QT interval and increase the risk of an abnormal heart rhythm. Elavil is not recommended during the acute recovery period after a heart attack.
