Test all patients for evidence of current or prior hepatitis B virus (HBV) infection before initiating treatment with EPCLUSA. HBV reactivation has been reported in HCV/HBV coinfected patients who were undergoing or had completed treatment with HCV direct acting antivirals and were not receiving HBV antiviral therapy. Some cases have resulted in fulminant hepatitis, hepatic failure, and death. Monitor HCV/HBV coinfected patients for hepatitis flare or HBV reactivation during HCV treatment and post-treatment follow-up. Initiate appropriate patient management for HBV infection as clinically indicated.

Hepatitis B virus (HBV) reactivation has been reported, in some cases resulting in fulminant hepatitis, hepatic failure, and death.

About Epclusa

Overview

Epclusa is a prescription medicine containing sofosbuvir and velpatasvir. It treats chronic hepatitis C infection caused by virus types 1 through 6 in adults and children aged 3 years and older, with or without compensated cirrhosis. For people with decompensated cirrhosis, it is used with ribavirin. Its two antiviral ingredients block proteins the hepatitis C virus needs to multiply.

What it treats

- Chronic hepatitis C infection caused by HCV genotypes 1, 2, 3, 4, 5 or 6 in adults and children aged 3 years and older without cirrhosis or with compensated cirrhosis. - Chronic hepatitis C caused by these genotypes in adults and children aged 3 years and older with decompensated cirrhosis, in combination with ribavirin.

Safety information

Side effects

Common side effects of Epclusa include headache and tiredness in adults and children aged 6 years and older. In children younger than 6 years, common reactions include vomiting and spitting up the medicine. When Epclusa is used with ribavirin in adults with decompensated cirrhosis, common side effects include tiredness, anemia (low red blood cell levels), nausea, headache, difficulty sleeping and diarrhea. Serious risks that warrant contacting a doctor include: • A dangerously slow heartbeat when Epclusa is taken with amiodarone. The risk is especially important in people also taking beta blockers or who have heart conditions or advanced liver disease. Taking Epclusa with amiodarone is not recommended; contact your doctor if you take amiodarone. • Reactivation of hepatitis B, which can cause a flare-up of liver inflammation. All patients should be tested for current or past hepatitis B before treatment. People with both hepatitis B and hepatitis C need monitoring during treatment and follow-up.

When to call your doctor

- Seek medical attention immediately for a slow heartbeat, especially if you also take amiodarone.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Epclusa must not be used with ribavirin in anyone who cannot take ribavirin. The ribavirin prescribing information lists those contraindications. All patients should be tested for current or past hepatitis B infection before starting treatment. People with both hepatitis B and hepatitis C need monitoring during treatment and follow-up because hepatitis B can reactivate and liver inflammation can flare. Taking Epclusa with amiodarone is not recommended because a serious slow heartbeat with symptoms may occur. The risk is of particular concern in people also taking beta blockers, or those with heart conditions or advanced liver disease. If no suitable alternative treatment is available, heart monitoring is recommended.

Interactions

Tell your doctor about all medicines and supplements you take. Rifampin, St. John’s wort and carbamazepine can lower the levels of sofosbuvir and/or velpatasvir in the body; taking them with Epclusa is not recommended. Other medicines that increase the activity of certain drug transporters or liver enzymes may have the same effect, so your doctor should check for interactions before treatment. Clearing hepatitis C with antiviral treatment may change liver function and affect how other medicines work. Your doctor may need to monitor blood glucose or INR (a measure of blood clotting) more frequently and adjust other medicines.

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
