Important Differences among Lamivudine-Containing Products

EPIVIR tablets and oral solution (used to treat HIV-1 infection) contain a higher dose of the active ingredient (lamivudine) than EPIVIR-HBV tablets and oral solution (used to treat chronic HBV infection). Patients with HIV-1 infection should receive only dosage forms appropriate for treatment of HIV-1.

About Epivir

Overview

Epivir contains lamivudine, an antiretroviral medicine. It is used with other antiretroviral medicines to treat human immunodeficiency virus type 1 (HIV-1) infection. It works by blocking an enzyme called reverse transcriptase.

What it treats

Treatment of human immunodeficiency virus type 1 (HIV-1) infection in combination with other antiretroviral medicines. This product is intended for HIV-1, not hepatitis B virus (HBV) infection.

Safety information

Side effects

Commonly reported side effects in adults include headache, nausea, feeling unwell, tiredness, nasal symptoms, diarrhoea and cough. In children, the most commonly reported side effects are fever and cough. Serious reactions include worsening of hepatitis B, a build-up of lactic acid in the blood (lactic acidosis), severe liver enlargement with fat build-up, inflammation of the pancreas (pancreatitis), and immune reconstitution syndrome. Contact a doctor if you suspect a serious reaction. Lactic acidosis and severe liver enlargement with fat build-up have included fatal cases. Particular caution is needed in children with a history of pancreatitis or other significant risk factors for it.

Patients with HIV-1 infection should receive only dosage forms of EPIVIR appropriate for treatment of HIV-1.

Before taking this medicine

Do not take Epivir if you have previously had an allergic reaction to lamivudine. Caution is needed in people with both HIV-1 and hepatitis B, as hepatitis B virus resistant to lamivudine has been reported during treatment. Children with a history of pancreatitis or significant risk factors for it also require caution; treatment may need to be stopped if clinically appropriate. Lactic acidosis (a buildup of lactic acid in the blood) and severe liver enlargement with fat accumulation, including fatal cases, have been reported with nucleoside analogue medicines. Immune reconstitution syndrome, an inflammatory response as the immune system recovers, has been reported with combination antiretroviral treatment. Women with HIV should not breastfeed because of the risk of transmitting HIV.

Interactions

Sorbitol may reduce lamivudine levels in the body. Tell your doctor if you use products containing sorbitol. Regular use together should be avoided when possible.

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
