Drug Interactions
Co-administration of certain drugs that are metabolized by human CYP3A4 enzymes are contraindicated with TOLSURA because plasma concentrations of such drugs are increased.
Co-administration with colchicine, fesoterodine and solifenacin is contraindicated in subjects with varying degrees of renal or hepatic impairment.
Co-administration with eliglustat is contraindicated in poor or intermediate metabolizers of CYP2D6 and in subjects taking strong or moderate CYP2D6 inhibitors.
Increased plasma concentrations of some of these drugs can lead to QT prolongation and ventricular tachyarrhythmias including occurrences of torsades de pointes, a potentially fatal arrhythmia.
About Itraconazole
Overview
Itraconazole is an antifungal medicine taken by mouth as capsules. The supplied TOLSURA label lists treatment of blastomycosis and histoplasmosis in adults, and aspergillosis when amphotericin B cannot be tolerated or has not worked. It acts as an azole antifungal; the supplied extract does not explain its action in more detail. TOLSURA is not interchangeable with other itraconazole products, so these label details should not be assumed to apply to all itraconazole capsules.
What it treats
Used in adults with or without weakened immune systems to treat: - Blastomycosis affecting the lungs or other parts of the body. - Histoplasmosis, including chronic lung disease with cavities and infection spread throughout the body that does not involve the membranes around the brain and spinal cord. - Aspergillosis affecting the lungs or other parts of the body, when amphotericin B is not tolerated or has not worked.
Safety information
Side effects
Common side effects include nausea, vomiting, diarrhoea, abdominal pain, loss of appetite, rash, itching, swelling, headache, dizziness, sleepiness, tiredness, fever and feeling unwell. Other common effects include high blood pressure, low potassium, abnormal liver function, protein in the urine, reduced sex drive and difficulty getting or keeping an erection. Contact a doctor promptly for serious side effects, including heart failure, abnormal heart rhythms, liver problems, allergic reactions, nerve problems or hearing loss. Liver damage can be severe and has caused liver failure and death. Stop treatment and contact a doctor if signs of liver problems or hearing loss occur. Hearing loss may be temporary or permanent. Nerve problems have been reported with long-term treatment and need prompt assessment. Itraconazole can also cause pseudoaldosteronism, a condition involving new or worsening high blood pressure and abnormal blood tests. Blood pressure and potassium levels should be monitored. Taking itraconazole with certain other medicines can cause life-threatening abnormal heart rhythms or sudden death.

Before taking this medicine
Do not take itraconazole if you are allergic to itraconazole or are taking certain medicines that affect its breakdown or whose breakdown it affects. Some combinations can cause life-threatening abnormal heart rhythms or sudden death. Have your prescriber review your medicines before treatment. Serious liver damage, including liver failure and death, has been reported. Stop treatment if signs of liver problems occur and contact your prescriber. Itraconazole can cause new or worsening high blood pressure and abnormal laboratory results, including changes in potassium levels. Blood pressure and potassium levels should be monitored. Nerve problems have been reported during long-term treatment. New nerve-related symptoms need prompt assessment. Hearing loss, which may be temporary or permanent, has also been reported. Stop treatment if hearing loss occurs and contact your prescriber.
Interactions
Itraconazole is mainly broken down by an enzyme called CYP3A4. Other medicines that use this enzyme or change its activity may affect itraconazole levels in your body. Ask your doctor to review your other medicines for possible interactions.
