About KUVAN
Overview
KUVAN (sapropterin dihydrochloride) is an oral medicine used to lower blood phenylalanine levels in adults and children aged one month and older with BH4-responsive phenylketonuria (PKU). It must be used alongside a diet restricted in phenylalanine. KUVAN is a synthetic form of BH4, a substance that helps activate the enzyme that breaks down phenylalanine, lowering its level in some people with PKU.
What it treats
High blood phenylalanine levels due to tetrahydrobiopterin (BH4)-responsive phenylketonuria (PKU) in adults and children aged one month and older, used alongside a phenylalanine-restricted diet.
Safety information
Side effects
Common side effects include headache, runny or blocked nose, throat pain, diarrhea, vomiting and cough. Serious side effects and concerns include: - Severe allergic reactions, including anaphylaxis. Signs may include wheezing, difficulty breathing, coughing, low blood pressure, flushing, nausea or rash. Stop taking KUVAN and seek immediate medical help if these occur. Continue prescribed dietary protein and phenylalanine (Phe) restrictions. - Inflammation of the food pipe or stomach, which can lead to narrowing of the food pipe, ulcers or bleeding if untreated. Contact your doctor if you develop symptoms affecting your stomach or swallowing. - Low blood Phe levels. This has occurred during treatment, with an increased risk observed in children younger than 7 years. Regular blood tests and dietary management are needed to maintain Phe control and nutritional balance, especially in children. - Seizures, worsening seizures, overstimulation or irritability when sapropterin is taken with levodopa. Contact your doctor promptly if these occur. - Hyperactivity. Tell your doctor if this develops.

Before taking this medicine
The label lists no formal contraindications. However, KUVAN is not recommended for anyone who has previously had anaphylaxis (a severe allergic reaction) to it. If anaphylaxis occurs, stop KUVAN and seek immediate medical care. Signs may include wheezing, difficulty breathing, coughing, low blood pressure, flushing, nausea or rash. Continue prescribed dietary protein and phenylalanine (Phe) restrictions. KUVAN requires caution and monitoring for: • Inflammation of the food pipe or stomach, which can lead to narrowing, ulcers or bleeding if untreated. • Blood Phe levels that become too low or remain too high. Regular blood tests and dietary management are needed to maintain Phe control and nutritional balance, especially in children. • Changes in neurological symptoms when taken with levodopa, including seizures, overstimulation or irritability. • Hyperactivity. Not everyone with phenylketonuria (PKU) responds to KUVAN. Response generally cannot be predicted by laboratory testing and must be assessed through a treatment trial. Safety and effectiveness have not been established in newborns. Clinical studies in people with PKU did not include adults aged 65 or older, so it is unknown whether they respond differently from younger people.
Interactions
KUVAN (sapropterin dihydrochloride) may interact with: • Levodopa: Neurological events have been reported in patients taking sapropterin with levodopa for a condition other than PKU. Your doctor should monitor for changes in neurological status. • Medicines that inhibit folate synthesis, including methotrexate, valproic acid, phenobarbital and trimethoprim: These may reduce the body’s available BH4 and increase blood phenylalanine (Phe) levels. More frequent blood Phe monitoring may be needed. • Medicines that relax blood vessels through nitric oxide, including sildenafil, vardenafil and tadalafil: Taking these with KUVAN could lower blood pressure. This combination has not been evaluated in humans; blood pressure monitoring is recommended. Tell your doctor if you take any of these medicines.

Dosing

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
