OSPHENA is an estrogen agonist/antagonist with tissue selective effects. In the endometrium, OSPHENA has estrogen agonistic effects. There is an increased risk of endometrial cancer in a woman with a uterus who uses unopposed estrogens. Perform adequate diagnostic measures, including directed and random endometrial sampling when indicated, to rule out malignancy in postmenopausal women with undiagnosed persistent or recurring abnormal genital bleeding
Estrogen-alone therapy has an increased risk of stroke and deep vein thrombosis (DVT). OSPHENA 60 mg had cerebral thromboembolic and hemorrhagic stroke incidence rates of 1.13 and 3.39 per thousand women years, respectively vs. 3.15 and 0 per thousand women years, respectively with placebo. For deep vein thrombosis, the incidence rate for OSPHENA 60 mg is 2.26 per thousand women years (2 reported cases) vs. 3.15 per thousand women years (1 reported case) with placebo
About Osphena
Overview
Osphena contains ospemifene, a non-hormonal medicine. It treats moderate to severe pain during sex and vaginal dryness caused by menopausal changes to the tissues of the vulva and vagina. It works by binding to estrogen receptors, activating estrogen pathways in some tissues and blocking them in others.
What it treats
- Moderate to severe pain during sexual intercourse caused by menopausal changes to vulvar and vaginal tissue (vulvar and vaginal atrophy). - Moderate to severe vaginal dryness caused by vulvar and vaginal atrophy due to menopause.
Safety information
Side effects
Common side effects include hot flushes, vaginal discharge, muscle spasms, headache, excessive sweating, vaginal bleeding and night sweats. Serious risks include cardiovascular problems, blood clots in deep veins (DVT) or the lungs (PE), and cancer. Seek urgent medical attention if you suspect a blood clot or another serious cardiovascular problem. Contact your doctor about vaginal bleeding or any suspected serious side effect.

Before taking this medicine
Do not take Osphena (ospemifene) if you have: • Unexplained abnormal genital bleeding. • An estrogen-dependent tumor. • A blood clot in a deep vein (DVT) or the lungs (pulmonary embolism), or a history of either. • A current or past blood clot in an artery, including a stroke or heart attack. • An allergy to ospemifene or any ingredient in Osphena. Do not take Osphena if you are pregnant, think you may be pregnant, or may become pregnant. It may harm an unborn baby. Tell your prescriber if you have known or suspected breast cancer, a history of breast cancer, or severe liver impairment. The label also warns about the risk of blood clots in deep veins and the lungs.
Interactions
Do not use Osphena with estrogens or medicines that act as estrogen agonists/antagonists. Do not use it with fluconazole, which increases ospemifene levels in the blood, or rifampin, which decreases them. Tell your doctor if you take any of these medicines.
