FDA boxed warningRisk of thyroid c-cell tumors

In rodents, semaglutide causes dose-dependent and treatment-duration-dependent thyroid C-cell tumors at clinically relevant exposures. It is unknown whether RYBELSUS and OZEMPIC tablets cause thyroid C-cell tumors, including medullary thyroid carcinoma (MTC), in humans as human relevance of semaglutide-induced rodent thyroid C-cell tumors has not been determined.

RYBELSUS and OZEMPIC tablets are contraindicated in patients with a personal or family history of MTC or in patients with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Counsel patients regarding the potential risk for MTC with the use of RYBELSUS or OZEMPIC tablets and inform them of symptoms of thyroid tumors (e.g., a mass in the neck, dysphagia, dyspnea, persistent hoarseness). Routine monitoring of serum calcitonin or using thyroid ultrasound is of uncertain value for early detection of MTC in patients treated with RYBELSUS or OZEMPIC tablets.

About Rybelsus

Overview

Rybelsus is an oral tablet containing semaglutide, a GLP-1 receptor agonist. It is used alongside diet and exercise to improve blood sugar control in adults with type 2 diabetes. It is also used to reduce the risk of cardiovascular death, non-fatal heart attack or non-fatal stroke in adults with type 2 diabetes who are at high risk of these events. It works by increasing insulin release and reducing the release of glucagon, a hormone that raises blood sugar, when blood sugar is high.

What it treats

- To improve blood sugar control in adults with type 2 diabetes, alongside diet and exercise. - To reduce the risk of cardiovascular death, non-fatal heart attack or non-fatal stroke in adults with type 2 diabetes who are at high risk of these events.

Safety information

Side effects

Common side effects of Rybelsus include nausea, stomach pain, diarrhea, reduced appetite, vomiting and constipation. Serious side effects that warrant contacting a doctor include inflammation of the pancreas (acute pancreatitis), complications of diabetes-related eye disease, low blood sugar, kidney injury due to dehydration, severe digestive problems, serious allergic reactions and gallbladder problems. Low blood sugar, including severe episodes, is more likely when Rybelsus is used with insulin or medicines that stimulate insulin release. The label also warns about a risk of thyroid C-cell tumors. Stop taking Rybelsus and seek medical help immediately if pancreatitis or a serious allergic reaction is suspected. Serious allergic reactions reported include anaphylaxis and angioedema, a type of swelling. People with a history of diabetes-related eye disease should be monitored. Kidney function may need monitoring if side effects cause dehydration. Rybelsus is not recommended for people with severe gastroparesis, a condition that slows stomach emptying. Stomach contents entering the lungs during general anesthesia or deep sedation has been reported with this class of medicines. Tell your healthcare team about any planned surgery or procedure.

Risk of thyroid c-cell tumors

When to call your doctor

- A lump in your neck, difficulty swallowing or breathing, or hoarseness that does not go away. - Severe stomach pain, vomiting or diarrhea. - A serious allergic reaction, including swelling under the skin.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not take Rybelsus if you or anyone in your family has had medullary thyroid carcinoma (MTC), if you have Multiple Endocrine Neoplasia syndrome type 2 (MEN 2), or if you have had a serious allergic reaction to semaglutide or any ingredient in the tablets. Rybelsus is not recommended for people with severe gastroparesis (severely delayed stomach emptying). Other risks requiring caution include inflammation of the pancreas, severe digestive side effects, kidney injury due to dehydration, and gallstones or gallbladder inflammation. Stop taking Rybelsus if pancreatitis is suspected or an allergic reaction occurs, and seek medical care. People with a history of diabetic retinopathy need monitoring. Taking Rybelsus with insulin or medicines that stimulate insulin release can increase the risk of low blood sugar, including severe episodes. Your prescriber may need to adjust those medicines. Kidney function should be monitored if side effects could cause dehydration. Tell your healthcare providers about any planned surgery or procedure. Inhalation of stomach contents into the lungs has been reported during general anesthesia or deep sedation in people taking this class of medicine. Breastfeeding is not recommended during treatment. Rybelsus should be stopped at least two months before a planned pregnancy.

Interactions

Rybelsus delays stomach emptying, which may affect other medicines taken by mouth. Tell your doctor about any oral medicines you take, especially those that need close monitoring or where small changes in drug levels can affect safety or effectiveness. Your doctor may need to arrange closer monitoring or additional blood tests.

Storage
