FDA boxed warningAsthma-related death
Long-acting beta 2 -adrenergic agonists (LABA), such as salmeterol, the active ingredient in SEREVENT DISKUS, as monotherapy (without inhaled corticosteroids [ICS]) increase the risk of asthma-related death. Data from a large placebo-controlled U.S. trial that compared the safety of salmeterol with placebo added to usual asthma therapy showed an increase in asthma-related deaths in subjects receiving salmeterol (13 deaths out of 13,176 subjects treated for 28 weeks on salmeterol versus 3 deaths out of 13,179 subjects on placebo). Use of background ICS was not required in this study. When LABA are used in fixed-dose combination with ICS, data from large clinical trials do not show a significant increase in the risk of serious asthma-related events (hospitalizations, intubations, death) compared with ICS alone.
Use of SEREVENT DISKUS for the treatment of asthma as monotherapy without a concomitant ICS is contraindicated. Use SEREVENT DISKUS only as additional therapy for patients with asthma who are currently taking but are inadequately controlled on an ICS. Do not use SEREVENT DISKUS for patients whose asthma is adequately controlled on low- or medium-dose ICS.

About Serevent
Overview
Serevent Diskus contains salmeterol xinafoate, a long-acting bronchodilator. It is used with an inhaled corticosteroid to treat asthma in people aged 4 years and older, to prevent exercise-induced airway narrowing in the same age group, and for maintenance treatment of airway narrowing associated with chronic obstructive pulmonary disease (COPD). It works by relaxing the muscles around the airways, helping them stay open. It is not for relief of sudden airway narrowing or breathing symptoms.
What it treats
- Asthma in patients aged 4 years and older, used with an inhaled corticosteroid. - Prevention of exercise-induced bronchospasm in patients aged 4 years and older. - Maintenance treatment of bronchospasm associated with chronic obstructive pulmonary disease (COPD). Not for relief of sudden episodes of bronchospasm.
Safety information
Side effects
Common side effects reported with Serevent include headache, flu, a blocked nose or sinuses, sore throat, nasal inflammation, and inflammation of the windpipe or airways in people with asthma. In people with COPD, common side effects include cough, headache, muscle or joint pain, throat irritation, and viral respiratory infections. Serious risks include sudden worsening of breathing immediately after use (paradoxical bronchospasm). If this occurs, stop using Serevent and seek urgent medical help. Low blood potassium and high blood sugar may also occur; contact your doctor if these are identified. Using Serevent alone for asthma, without an inhaled corticosteroid, increases the risk of asthma-related death and hospitalisation. People with asthma must also take an inhaled corticosteroid. Serevent does not treat sudden breathing symptoms. Do not use it with another long-acting beta agonist (LABA), as this can cause an overdose.

Before taking this medicine
Do not use Serevent Diskus: • For asthma unless you also use an inhaled corticosteroid. Using it alone for asthma increases the risk of asthma-related death and hospitalisation. • To treat sudden asthma or COPD symptoms, or severe attacks requiring intensive treatment. Do not start it when asthma or COPD is rapidly worsening. • If you have a severe allergy to milk proteins or an allergy to salmeterol or any other ingredient. • Alongside another medicine containing a long-acting beta₂-agonist (LABA), because of the risk of overdose. Use with caution if you have heart or blood vessel disease, a central nervous system disorder, a seizure disorder, an overactive thyroid, diabetes or ketoacidosis. It may cause low blood potassium or high blood sugar. People with liver impairment need monitoring for signs of increased exposure to the medicine. Serevent Diskus does not replace corticosteroids. If it causes sudden worsening of wheezing or breathing immediately after use, stop using it and seek medical help for alternative treatment.
Interactions
Tell your doctor if you take any of the following: • Strong CYP3A4 inhibitors, such as ritonavir or ketoconazole. Use with Serevent is not recommended because it may increase the risk of effects on the heart and blood vessels. • Monoamine oxidase inhibitors (MAOIs) or tricyclic antidepressants. These require extreme caution because they may increase salmeterol’s effects on blood vessels. • Beta-blockers. These may reduce Serevent’s airway-opening effect and cause severe narrowing of the airways. • Non–potassium-sparing diuretics (water tablets). Using these with Serevent may worsen low blood potassium or changes in the heart’s electrical activity.

Storage
Store below 25°C away from direct heat and sunlight. Do not pierce or burn the device, even when it is empty. Keep all medicines out of the sight and reach of children.
