Increased Mortality in Elderly Patients with Dementia-Related Psychosis
Elderly patients with dementia-related psychosis treated with antipsychotic drugs are at an increased risk of death. SEROQUEL XR is not approved for the treatment of patients with dementia-related psychosis.
Suicidal Thoughts and Behaviors
Antidepressants increased the risk of suicidal thoughts and behavior in children, adolescents, and young adults in short-term studies. These studies did not show an increase in the risk of suicidal thoughts and behavior with antidepressant use in patients over age 24; there was a reduction in risk with antidepressant use in patients aged 65 and older.
SEROQUEL XR is not approved for use in pediatric patients under ten years of age.

About Seroquel XR
Overview
Seroquel XR is an atypical antipsychotic medicine containing quetiapine fumarate in extended-release tablets. It is used to treat schizophrenia, manic or mixed episodes of bipolar I disorder, and depressive episodes of bipolar disorder. It is also used alongside antidepressants to treat major depressive disorder. Exactly how it works is unclear, but its effects may involve blocking certain receptors for the brain chemicals dopamine and serotonin.

Safety information
Side effects
Common side effects in adults include sleepiness, dry mouth, constipation, dizziness, increased appetite, indigestion, weight gain, tiredness, difficulty speaking clearly and a blocked nose. In children and adolescents, common side effects include sleepiness, dizziness, tiredness, increased appetite, nausea, vomiting, dry mouth, a fast heartbeat and weight gain. Serious risks that warrant contacting a doctor include suicidal thoughts or behaviours, particularly in adolescents and young adults; stroke; neuroleptic malignant syndrome; uncontrolled movements (tardive dyskinesia); seizures; heart rhythm changes (QT prolongation); low blood pressure and falls; and reduced white blood cell counts. Seek immediate medical help for suicidal thoughts or suspected stroke, seizures or neuroleptic malignant syndrome. Elderly patients with dementia-related psychosis have an increased risk of death and stroke. Other important effects include high blood sugar or diabetes, changes in blood fats, weight gain, cataracts, reduced thyroid function, increased prolactin levels, difficulty swallowing, impaired thinking or movement, and problems regulating body temperature. Contact your doctor if you develop increased thirst, frequent urination, increased hunger or weakness, which may indicate high blood sugar. Blood pressure may rise in children and adolescents. Symptoms can also occur when treatment is stopped. Monitoring may include blood sugar, blood fats, weight and eye examinations. Children and adolescents need blood pressure monitoring. Patients with a low white blood cell count or a history of low counts may need frequent blood tests during the first few months. Extra caution is needed in people with urinary retention, an enlarged prostate or constipation, and when taking other medicines with anticholinergic effects.

Before taking this medicine
Do not take Seroquel XR if you are allergic to quetiapine or any of its ingredients. Severe allergic reactions, including anaphylaxis, have been reported. Conditions that require caution or monitoring include: • Heart or blood vessel disease, including disease affecting blood flow to the brain, because of the risk of low blood pressure. • Dementia-related psychosis in older adults, as atypical antipsychotics have been associated with an increased risk of stroke and transient ischemic attack. • Diabetes or risk factors for diabetes. Blood glucose, blood fats and weight may need monitoring. • A low white blood cell count or a history of low white blood cells or neutrophils. Frequent blood counts are needed during the first few months of treatment. • Urinary retention, an enlarged prostate or constipation, or use of other medicines with anticholinergic effects. • Pregnancy, particularly exposure during the third trimester, which may cause movement-related or withdrawal symptoms in the newborn. Blood pressure should be monitored in children and adolescents. Eye lens examinations are recommended when treatment starts and every six months during long-term treatment because lens changes have been reported. Neuroleptic malignant syndrome requires immediate discontinuation and close monitoring; tardive dyskinesia may also require treatment to be stopped by the prescriber.
Interactions
Tell your doctor if you take ketoconazole, ritonavir, phenytoin, rifampin or the herbal supplement St. John’s wort. These can interact with Seroquel XR, and your prescriber may need to adjust your treatment. Also tell your doctor before stopping phenytoin, rifampin or St. John’s wort, as this may require a further adjustment.

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
