About Solu‑Cortef
Overview
Solu-Cortef is a glucocorticoid (corticosteroid) injection containing hydrocortisone sodium succinate. When oral treatment is not feasible, it is used for conditions including adrenal insufficiency, severe allergic conditions that have not responded to conventional treatment, and certain inflammatory disorders. It works by reducing inflammation.
What it treats
When treatment by mouth is not feasible, Solu-Cortef injection is used for conditions including: - Severe allergic conditions that have not responded to conventional treatment, including asthma and allergic dermatitis. - Severe skin diseases, including pemphigus and Stevens-Johnson syndrome. - Adrenal insufficiency, congenital adrenal hyperplasia, cancer-associated high blood calcium and thyroid inflammation without infection. - Ulcerative colitis and regional enteritis during critical periods. - Certain blood disorders, including autoimmune hemolytic anemia and pure red cell aplasia. - Leukemias and lymphomas, for palliative management. - Brain swelling associated with brain tumors or surgery. - Certain inflammatory eye conditions. - Nephrotic syndrome that is idiopathic or associated with lupus. - Certain lung diseases, including symptomatic sarcoidosis and idiopathic eosinophilic pneumonia. - Rheumatic and inflammatory conditions, including rheumatoid arthritis, acute gouty arthritis, ankylosing spondylitis, lupus, dermatomyositis, polymyositis and temporal arteritis.
Safety information
Side effects
Reported side effects include nausea, increased appetite, weight gain, fluid retention, headache, dizziness, trouble sleeping, mood changes, increased sweating, acne, rash, easy bruising, thinning skin and slow wound healing. Menstrual changes, muscle weakness and increased blood sugar may also occur. The label does not state how common these effects are. Serious reported side effects that warrant contacting a doctor include: • Severe allergic reactions, including anaphylaxis and swelling beneath the skin. • Heart rhythm problems, heart failure, cardiac arrest, blood clots, high blood pressure and fluid in the lungs. • Stomach or intestinal ulcers, bleeding or perforation, and inflammation of the pancreas. • Reduced resistance to infection. • Depression and other significant mood, personality or mental health changes; seizures; and increased pressure inside the skull. • Glaucoma, cataracts and other eye problems that can affect vision. • Bone thinning, fractures, bone tissue death, tendon rupture and loss of muscle mass. • Reduced adrenal gland function, slowed growth in children, diabetes becoming apparent or increased need for diabetes medicines. • Potassium loss and salt and fluid retention. Injection-site infections have also been reported following non-sterile administration.
When to call your doctor
If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
Solu‑Cortef must not be used in people with a systemic fungal infection or a known allergy to the medicine or any of its ingredients. It must not be given by injection into a muscle in people with idiopathic thrombocytopenic purpura (ITP). It must not be injected into the fluid surrounding the spinal cord (intrathecal administration). Severe medical events have been reported with this route.
Interactions
Solu-Cortef can interact with other medicines. Tell your doctor if you use: • Amphotericin B or medicines that lower potassium, such as diuretics: potassium may fall too low. Heart enlargement and heart failure have been reported with amphotericin B and hydrocortisone together. • Digitalis medicines: low potassium may increase the risk of an irregular heartbeat. • Warfarin: its blood-thinning effect may change, so frequent blood-clotting tests may be needed. • Diabetes medicines: corticosteroids can raise blood sugar, so your doctor may need to adjust treatment. • Aspirin or other nonsteroidal anti-inflammatory drugs (NSAIDs): stomach and intestinal side effects may increase. Aspirin needs particular caution if you have low levels of the clotting protein prothrombin. • Cyclosporine: the effects of both medicines may increase. Seizures have been reported with combined use. • Anticholinesterase medicines for myasthenia gravis: combined use may cause severe weakness. • Ketoconazole or macrolide antibiotics, such as erythromycin or troleandomycin: these may increase corticosteroid levels and side effects. • Barbiturates, phenytoin, carbamazepine or rifampin: these may speed up corticosteroid breakdown. • Estrogens, including oral contraceptives: these may increase corticosteroid effects. • Cholestyramine: this may speed up corticosteroid removal from the body. • Isoniazid: its blood levels may decrease. • Aminoglutethimide: this may reduce corticosteroid-related suppression of the adrenal glands. Discuss vaccinations with your doctor. Prolonged corticosteroid treatment may reduce vaccine responses, and live vaccines may carry an increased risk because some vaccine organisms can multiply more readily. Routine vaccinations should be postponed until corticosteroid treatment has ended, if possible. Also tell the clinician before skin testing, as corticosteroids may suppress test reactions.

Storage
Store in the original carton below 25°C, protected from light, unless the pack states refrigeration. Do not freeze. Keep all medicines out of the sight and reach of children.
