About Adempas
Overview
Adempas (riociguat) is a prescription tablet used to treat adults with certain types of pulmonary hypertension. It treats chronic thromboembolic pulmonary hypertension (CTEPH) that persists or returns after surgery, or cannot be treated with surgery, to improve exercise capacity and functional ability. It also treats pulmonary arterial hypertension (PAH) to improve exercise capacity and functional ability and delay worsening of the condition. It works by stimulating an enzyme called soluble guanylate cyclase, increasing a chemical signal that helps blood vessels widen.
What it treats
- Chronic thromboembolic pulmonary hypertension (CTEPH, WHO Group 4) in adults when it persists or returns after surgery, or cannot be treated surgically, to improve exercise capacity and functional class. - Pulmonary arterial hypertension (PAH, WHO Group 1) in adults, to improve exercise capacity and functional class and delay clinical worsening.
Safety information
Side effects
Common side effects include headache, indigestion or stomach inflammation, dizziness, nausea, diarrhea, low blood pressure, vomiting, anemia (low red blood cells), acid reflux and constipation. Contact a doctor promptly for low blood pressure that causes symptoms, bleeding, or pulmonary edema (fluid in the lungs). Pulmonary edema can occur in people with pulmonary veno-occlusive disease; if this condition is confirmed, treatment must be stopped. Adempas can harm an unborn baby. Contact your doctor immediately if you are pregnant or think you may be pregnant.

Before taking this medicine
Do not take Adempas (riociguat) if you are pregnant, have pulmonary hypertension associated with idiopathic interstitial pneumonias (PH-IIP), or use nitrates, nitric oxide donors in any form, phosphodiesterase (PDE) inhibitors or other soluble guanylate cyclase (sGC) stimulators. Adempas is not recommended for people with creatinine clearance below 15 mL/min, those on dialysis, or those with severe liver impairment (Child-Pugh C). Do not breastfeed while taking Adempas. Warnings include low blood pressure that causes symptoms, bleeding, and fluid build-up in the lungs in people with pulmonary veno-occlusive disease. If pulmonary veno-occlusive disease is confirmed, treatment must be discontinued. Tell your prescriber if you smoke or stop smoking, as this may affect your prescribed treatment.
Interactions
Tell your doctor if you take medicines that strongly inhibit CYP enzymes and the P-gp/BCRP transporters. With these medicines, monitoring for low blood pressure is recommended. Take antacids at least 1 hour before or after Adempas.

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
