About Afinitor

Overview

Afinitor tablets contain everolimus, a medicine called a kinase inhibitor. It is used for certain advanced breast cancers in combination with exemestane, certain progressive neuroendocrine tumors of the pancreas, gut or lung, and advanced kidney cancer after specific earlier treatments have failed. It also treats kidney angiomyolipomas in adults with tuberous sclerosis complex (TSC) who do not need immediate surgery, and TSC-associated subependymal giant cell astrocytomas in adults and children aged 1 year and older when treatment is needed but surgery cannot provide a cure. Everolimus blocks a protein called mTOR, which is involved in cell growth and is overactive in several cancers and TSC.

What it treats

- Advanced hormone receptor-positive, HER2-negative breast cancer in postmenopausal women, together with exemestane after treatment with letrozole or anastrozole has failed. - Progressive neuroendocrine tumors of the pancreas in adults, or progressive, well-differentiated, non-functional neuroendocrine tumors of the gastrointestinal tract or lungs in adults, when unresectable, locally advanced or metastatic. Afinitor is not indicated for functional carcinoid tumors. - Advanced renal cell carcinoma in adults after treatment with sunitinib or sorafenib has failed. - Renal angiomyolipoma in adults with tuberous sclerosis complex (TSC) who do not need immediate surgery. - Subependymal giant cell astrocytoma (SEGA) in adults and children aged 1 year and older with TSC, when treatment is needed but surgery cannot completely remove the tumor.

Safety information

Side effects

Common side effects of Afinitor include mouth sores or inflammation, infections, rash, tiredness, weakness, diarrhea, swelling, abdominal pain, nausea, fever, cough, headache and decreased appetite. The most common side effects vary by the condition being treated. Contact your doctor promptly if you develop signs of lung inflammation, infection, a severe allergic reaction or angioedema (swelling beneath the skin). Lung inflammation can occur without an infection. Taking an ACE inhibitor at the same time may increase the risk of angioedema. Other serious risks include kidney failure, impaired wound healing, changes in blood sugar or blood fats, reduced blood cell production and severe reactions related to radiation treatment. Your doctor will monitor kidney function, blood sugar, blood fats and blood cell counts before and during treatment. Report mouth sores and wound-healing problems to your doctor. Serious reactions may require treatment to be paused or stopped by your prescriber.

When to call your doctor

- Seek medical attention immediately for fever or a new or worsening cough. - Seek medical attention immediately for severe swelling or a severe allergic reaction.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not take Afinitor if you have a clinically significant allergy to everolimus or other rapamycin derivatives. Conditions requiring caution include lung inflammation not caused by infection, infections, kidney problems, liver impairment, high blood glucose or lipid levels, and low blood cell counts. Kidney function, blood glucose, lipids and blood cell counts need monitoring before and during treatment. Older patients require monitoring for side effects. Taking an ACE inhibitor with Afinitor may increase the risk of angioedema, a serious swelling reaction. Clinically significant allergic reactions or angioedema require permanent discontinuation. Afinitor can impair wound healing, so treatment must be withheld around planned surgery. Mouth inflammation can occur; the label recommends starting dexamethasone alcohol-free mouthwash when treatment begins. Severe radiation reactions may also occur. Avoid live vaccines and close contact with people who have received live vaccines. Recommended childhood vaccinations should be completed before treatment starts. Afinitor can harm an unborn baby. Patients who could conceive a child should use effective contraception.

Interactions

Tell your doctor about all medicines and supplements you take, especially cannabidiol oral solution. Avoid taking Afinitor with medicines that strongly inhibit both CYP3A and P-gp, which help process and transport medicines in the body. Medicines that moderately inhibit these pathways, medicines that strongly increase their activity, and cannabidiol oral solution may require your prescriber to adjust your Afinitor treatment. Do not change your dose yourself.

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
