About ALKINDI SPRINKLE
Overview
ALKINDI SPRINKLE contains hydrocortisone, a corticosteroid also known as cortisol, as oral granules contained within capsules. It is used as replacement therapy in children with adrenocortical insufficiency, when the adrenal glands do not make enough cortisol. It works by replacing cortisol and affects metabolism and the body’s immune responses.

Side effects
Common side effects of corticosteroids include fluid retention, changes in how the body handles blood sugar, raised blood pressure, changes in behaviour or mood, increased appetite and weight gain. Serious risks include adrenal crisis, which can be fatal. This may occur with insufficient treatment, sudden stopping of treatment, switching hydrocortisone products, or stress such as infection or surgery. Do not stop treatment suddenly. Seek urgent medical advice if the patient is vomiting, severely ill or unable to take oral medicines. Contact a doctor if signs of infection, severe mood or behaviour changes, depression, hallucinations, confusion, blurred vision or other vision changes develop. Doses above replacement needs can suppress the immune system and increase the risk of new infections or worsening existing infections. Other serious risks include slowed growth, Cushing’s syndrome from prolonged excessive corticosteroid exposure, reduced bone density and osteoporosis. Prolonged high-dose use has been linked to cataracts, glaucoma and a retinal condition called central serous chorioretinopathy. Gastrointestinal complications are more likely in people with certain digestive disorders, and warning symptoms may be masked. Doses above replacement needs also carry a risk of Kaposi’s sarcoma.

Before taking this medicine
Do not take ALKINDI SPRINKLE if you are allergic to hydrocortisone or any of its ingredients. Severe allergic reactions, including anaphylaxis, have occurred with corticosteroids. Too little treatment or stopping treatment suddenly can cause adrenal crisis, which can be fatal. Infections, surgery and other physical stress can also trigger adrenal crisis. Seek medical advice during these situations, or if vomiting, severe illness or inability to take oral medicines occurs. Do not stop treatment without medical advice. Treatment above the body’s replacement needs can suppress the immune system and increase the risk of new infections or worsening existing infections. Prolonged excessive treatment can slow growth in children, cause Cushing’s syndrome and reduce bone density. Caution and monitoring are needed for mood or behavioural changes, including depression, mania, hallucinations or confusion. Seek medical advice if these occur. Prolonged high-dose treatment has also been linked to cataracts, glaucoma and other eye problems; report blurred vision or other visual changes. Certain gastrointestinal disorders can increase the risk of complications, and corticosteroids may mask their signs and symptoms.
Interactions
Discuss these interactions with your doctor when taking ALKINDI SPRINKLE: • Grapefruit juice and medicines such as itraconazole, posaconazole, voriconazole, erythromycin, clarithromycin and ritonavir may increase hydrocortisone levels and the risk of side effects. • Phenytoin, carbamazepine, oxcarbazepine, rifampicin, rifabutin, phenobarbital, primidone, efavirenz and nevirapine may lower hydrocortisone levels, increasing the risk of adrenal crisis. • Oral estrogen and estrogen-containing contraceptive pills may reduce how well ALKINDI SPRINKLE works. • Diabetes medicines may need adjustment by your doctor because hydrocortisone above the body’s normal replacement needs can raise blood sugar. • Warfarin’s effect may change, usually becoming weaker. Blood-clotting tests are needed to monitor this interaction. • Cyclosporine and hydrocortisone may increase each other’s effects. Seizures have been reported with this combination, and monitoring is needed. • Nonsteroidal anti-inflammatory drugs (NSAIDs), including aspirin, increase the risk of stomach and intestinal side effects. Aspirin needs particular caution in people with low levels of the clotting protein prothrombin. Hydrocortisone may lower blood levels of salicylates such as aspirin; stopping hydrocortisone may increase the risk of salicylate toxicity. Your prescriber may need to adjust treatment or arrange monitoring if these are used together.

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
