FDA boxed warningDepression and suicidality in patients with huntington’s disease

AUSTEDO XR and AUSTEDO can increase the risk of depression and suicidal thoughts and behavior (suicidality) in patients with Huntington’s disease. Anyone considering the use of AUSTEDO XR or AUSTEDO must balance the risks of depression and suicidality with the clinical need for treatment of chorea. Closely monitor patients for the emergence or worsening of depression, suicidality, or unusual changes in behavior. Patients, their caregivers, and families should be informed of the risk of depression and suicidality and should be instructed to report behaviors of concern promptly to the treating physician.

Particular caution should be exercised in treating patients with a history of depression or prior suicide attempts or ideation, which are increased in frequency in Huntington’s disease. AUSTEDO XR and AUSTEDO are contraindicated in patients who are suicidal, and in patients with untreated or inadequately treated depression

About Austedo

Overview

Austedo is a prescription medicine containing deutetrabenazine. It is used in adults to treat chorea associated with Huntington’s disease and tardive dyskinesia. Its exact way of working is not fully understood, but it is thought to reduce stores of chemical messengers, including dopamine, in nerve endings by blocking a protein called VMAT2.

Side effects

Common side effects of Austedo in patients with Huntington’s disease include sleepiness, diarrhea, dry mouth and tiredness. In patients with tardive dyskinesia, common side effects include inflammation of the nose and throat and difficulty sleeping. Sleepiness may affect your ability to drive or operate complex machinery. Serious risks include depression and suicidal thoughts or behavior in patients with Huntington’s disease, changes in the heart’s electrical activity called QT prolongation, neuroleptic malignant syndrome, agitation, restlessness or an inability to sit still, Parkinson-like movement problems, and increased prolactin levels. Contact your doctor promptly if you experience serious side effects, particularly depression, suicidal thoughts, agitation, restlessness or new movement problems. Austedo should be avoided in people with congenital long QT syndrome or heart rhythm disorders associated with a prolonged QT interval. If neuroleptic malignant syndrome occurs, treatment must be stopped.

When to call your doctor

- Seek medical attention immediately for suicidal thoughts or behavior. - Contact your doctor right away for new or worsening depression or unusual changes in behavior. - Contact your doctor right away for agitation or restlessness.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not take Austedo if you: • Have Huntington’s disease and are suicidal, or have depression that is untreated or not adequately treated. • Have liver impairment. • Take reserpine, or stopped taking it less than 20 days ago. • Take a monoamine oxidase inhibitor (MAOI), or stopped taking one less than 14 days ago. • Take tetrabenazine or valbenazine. Avoid Austedo if you have congenital long QT syndrome or a heart rhythm disorder associated with a prolonged QT interval. Austedo may cause neuroleptic malignant syndrome (NMS), which requires stopping treatment. It may also cause agitation, restlessness, an urge to keep moving (akathisia), or Parkinson-like symptoms; these may require your prescriber to adjust or stop treatment. Austedo can cause sleepiness or sedation that may impair driving or operating complex machinery. If you are pregnant or planning pregnancy, discuss this with your prescriber: animal studies suggest Austedo may harm an unborn baby.

Interactions

Tell your doctor if you take a strong CYP2D6 inhibitor, as this limits how much Austedo can be prescribed. Alcohol and other medicines that cause sedation may increase sleepiness when used with Austedo.

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
