About Bumex

Overview

Bumex tablets contain bumetanide, a loop diuretic often called a water pill. They treat fluid buildup (edema) associated with congestive heart failure, liver disease and kidney disease, including nephrotic syndrome. Bumetanide increases urine output to help the body remove excess fluid.

Safety information

Side effects

The most frequently reported side effects of Bumex include muscle cramps, dizziness, low blood pressure, headache and nausea. Other reported effects include itching, hives, rash, weakness, abdominal pain, vomiting, diarrhea, fatigue, and joint or muscle pain. Serious reported effects include severe skin reactions such as Stevens–Johnson syndrome and toxic epidermal necrolysis, kidney failure, dehydration, hearing impairment, and encephalopathy (impaired brain function) in people with pre-existing liver disease. Contact a doctor if you develop a skin reaction, hearing changes, chest pain or changes in brain function. Bumex can also cause low blood levels of potassium, sodium and chloride, as well as increased uric acid, blood sugar and markers of kidney function. Low platelet counts and other blood-test abnormalities have also been reported. These changes may need medical assessment even when they do not cause noticeable symptoms.

When to call your doctor

- Seek medical attention immediately for chest pain. - Get immediate help for a severe skin rash. - Seek urgent care for severe dizziness, weakness or muscle cramps. - Get immediate help for new confusion, especially if you have liver disease. - Seek urgent medical advice for new or worsening hearing problems.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not take Bumex if you are unable to produce urine (anuria), are allergic to bumetanide, are in a coma caused by liver disease, or have severe depletion of electrolytes (body salts). Liver-related coma or severe electrolyte depletion must improve or be corrected before treatment. In people with progressive kidney disease, treatment must be stopped if blood urea nitrogen or creatinine levels rise markedly, or if urine output becomes very low (oliguria).

Interactions

Bumex should generally not be taken with lithium because it reduces lithium clearance by the kidneys, increasing the risk of lithium toxicity. Medicines known to harm the kidneys should also be avoided with Bumex. Probenecid should not be taken with Bumex, and indomethacin is not recommended alongside it, because both can reduce its effects. Bumex may increase the effects of blood pressure medicines. Your doctor may need to adjust those medicines. Human interaction studies found no effect on digoxin blood levels, warfarin metabolism or blood prothrombin activity, a measure related to clotting.

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
