About Cordarone IV
Overview
Cordarone IV contains amiodarone hydrochloride, an antiarrhythmic medicine used to treat serious heart rhythm disturbances. It is used to start treatment and help prevent frequently recurring ventricular fibrillation and hemodynamically unstable ventricular tachycardia, dangerous rhythms in the heart’s lower chambers, when other treatments have not worked. It works by affecting electrical signals in the heart, slowing their passage through the node that connects the upper and lower chambers.
What it treats
- Frequently recurring ventricular fibrillation when other treatments have not worked, to start treatment and help prevent recurrence. - Hemodynamically unstable ventricular tachycardia (a rapid heart rhythm that disrupts blood circulation) when other treatments have not worked, to start treatment and help prevent recurrence.
Safety information
Side effects
Low blood pressure is among the most common side effects leading to intravenous amiodarone treatment being stopped. Other reactions commonly leading to treatment being stopped include a dangerously fast heartbeat, cardiac arrest and cardiogenic shock, when the heart cannot pump enough blood. These reactions are serious and require immediate medical attention. Other serious side effects include a slow heartbeat, blocked electrical signals in the heart, dangerous abnormal heart rhythms including torsade de pointes, heart failure, liver injury or abnormal liver tests, lung injury, thyroid injury and allergic reactions. Contact a doctor immediately if any of these occur.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
Do not use if you have a known allergy to amiodarone hydrochloride, iodine or any ingredient in the medicine; cardiogenic shock (when the heart cannot pump enough blood); or a markedly slow heart rate. It must not be used in people with second- or third-degree atrioventricular (AV) block unless a functioning pacemaker is available. Low blood pressure, a slow heart rate and AV block are important treatment risks. These may require the clinician to slow the infusion; a slow heart rate or AV block may require stopping treatment. During pregnancy, use only if the potential benefit to the mother justifies the risk to the fetus. Breastfeeding should be discontinued.
Interactions
Amiodarone can interact with medicines, supplements and grapefruit juice. Discuss these with your doctor: • Medicines that prolong the QT interval (part of the heart’s electrical rhythm), including certain medicines for abnormal heart rhythms, lithium, some antidepressants, certain antibiotics, azole antifungals and some inhaled anaesthetics. These combinations should be avoided because they increase the risk of a dangerous heart rhythm called torsade de pointes. • Medicines that slow the heart, including digoxin, beta blockers, verapamil, diltiazem, clonidine and ivabradine. Combining them with amiodarone can cause a very slow heartbeat or blocked electrical signals in the heart. Heart-rate monitoring is needed. • Grapefruit juice, cimetidine, certain fluoroquinolone or macrolide antibiotics, and azole antifungals. These can increase amiodarone levels and should be avoided during treatment. • St John’s wort and cholestyramine, which can lower amiodarone levels. • Warfarin. Amiodarone can increase its blood-thinning effect and cause serious or fatal bleeding. Your doctor will need to monitor your INR and adjust treatment. • Digoxin, cyclosporine, phenytoin and statins such as simvastatin, lovastatin and atorvastatin. Amiodarone can increase their levels. Your doctor may need to adjust treatment or monitor drug levels, kidney function or signs of toxicity. • Other heart-rhythm medicines, including quinidine, procainamide and flecainide. Combined use is reserved for people whose condition does not respond to one medicine alone and requires careful monitoring.

Storage
Store in the original carton below 25°C, protected from light. Do not freeze, and use immediately once opened. Keep all medicines out of the sight and reach of children.
