The applicability of the CAST results to other populations (e.g., those without recent myocardial infarction) is uncertain. Considering the known proarrhythmic properties of Norpace or Norpace CR and the lack of evidence of improved survival for any antiarrhythmic drug in patients without life-threatening arrhythmias, the use of Norpace or Norpace CR as well as other antiarrhythmic agents should be reserved for patients with life-threatening ventricular arrhythmias.
About Norpace
Overview
Norpace contains disopyramide, an antiarrhythmic medicine used to treat documented abnormal heart rhythms that a doctor judges to be life-threatening, such as sustained ventricular tachycardia. It works by changing the electrical activity of heart cells to help control abnormal rhythms. Treatment should begin in hospital because Norpace can itself cause abnormal heart rhythms. Antiarrhythmic medicines have not been shown to improve survival in people with ventricular arrhythmias.
What it treats
Documented ventricular arrhythmias judged by a physician to be life-threatening, such as sustained ventricular tachycardia.
Safety information
Side effects
Common side effects include dry mouth, difficulty starting urination and constipation. Other reported effects include blurred vision, dry eyes, nose or throat, frequent or urgent urination, nausea, abdominal pain, bloating, gas, dizziness, tiredness, muscle weakness, headache and general aches. These effects may persist or become severe. Contact a doctor promptly if you cannot pass urine, or develop fainting, chest pain, shortness of breath, swelling or weight gain. Serious reported effects include low blood pressure, new or worsening heart failure, and disturbances in the heart’s electrical conduction. Other serious reported reactions include low blood sugar, liver problems with jaundice, low blood cell counts, breathing difficulty, fever and severe mental changes, including acute psychosis. Contact a doctor promptly if these occur.

Before taking this medicine
Do not take Norpace if you have cardiogenic shock, pre-existing second- or third-degree heart block without a pacemaker, congenital QT prolongation (a heart rhythm abnormality present from birth), or a known allergy to disopyramide.
Interactions
Clarithromycin and erythromycin have caused life-threatening interactions with disopyramide. Medicines that block the liver enzyme CYP3A4, such as ketoconazole, may raise disopyramide levels and could cause potentially fatal interactions. Phenytoin and other medicines that increase liver enzyme activity may lower disopyramide levels, making treatment less effective. Blood-level monitoring is recommended when these medicines are used together. Other heart rhythm medicines, including quinidine, procainamide, lidocaine and propranolol, may cause excessive changes in the heart’s electrical conduction when combined with disopyramide. Disopyramide should not be taken within 48 hours before or 24 hours after verapamil. Tell your doctor if you take verapamil so they can manage the timing safely.

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
