Before taking this medicine

The supplied selegiline label lists the following medicines as contraindicated: opioids such as meperidine, tramadol and methadone; other MAO inhibitors, including selective MAO-B inhibitors and linezolid; dextromethorphan; St. John’s wort; and cyclobenzaprine. These combinations can cause serious reactions, including potentially fatal serotonin syndrome, dangerously high blood pressure, or psychosis and unusual behaviour. At least 14 days must pass after stopping treatment before starting an opioid or another MAO inhibitor. Antidepressants require caution because combined use may cause serotonin syndrome. Other warnings include high blood pressure, low blood pressure or dizziness on standing, falling asleep during everyday activities, new or worsening involuntary movements, hallucinations, psychotic-like behaviour, and compulsive urges. Abrupt discontinuation may cause a very high temperature and confusion. The supplied label does not recommend use in severe kidney impairment. Animal studies suggest possible harm to an unborn baby. It also warns of irritation inside the mouth and increased risk in people with phenylketonuria. These warnings come from the supplied ZELAPAR label; their applicability to the Eldepryl product should be confirmed with the prescriber or pharmacist.
