Before taking this medicine

Do not take Zelapar with opioid medicines such as meperidine, tramadol or methadone; other monoamine oxidase inhibitors (MAOIs), including selective MAO-B inhibitors and linezolid; St. John’s wort; cyclobenzaprine; or dextromethorphan. These combinations can cause serious reactions, including potentially fatal serotonin syndrome, a dangerous rise in blood pressure, or psychosis and unusual behaviour. At least 14 days must pass after stopping Zelapar before starting an opioid or another MAOI. Zelapar is not recommended for people with severe kidney impairment. It may harm an unborn baby, based on animal studies, and carries an increased risk for people with phenylketonuria. Use with antidepressants may cause serotonin syndrome. Other concerns include high blood pressure, low blood pressure when standing, falling asleep during daily activities, new or worsening involuntary movements, hallucinations or psychotic-like behaviour, impulse-control problems or compulsive behaviour, and irritation inside the mouth. Stopping suddenly may cause a very high fever and confusion.
