Safety information

Side effects

Common side effects include constipation, dizziness, nausea, headache, tiredness, swelling, and low blood pressure. Shortness of breath, a slow heartbeat, rash, and flushing have also been reported. Contact a doctor if you develop a slow heartbeat, shortness of breath, swelling, or fainting. Serious side effects include heart failure, fluid in the lungs, severe low blood pressure, and heart block (disrupted electrical signals in the heart). Severe low blood pressure or complete heart block requires immediate emergency treatment. Increased liver enzymes and, infrequently, a temporary loss of bowel movement without a physical blockage have also been reported. Severe skin reactions, including Stevens–Johnson syndrome, have been reported, although a causal link to verapamil is uncertain. Seek urgent medical attention for a severe skin reaction.

When to call your doctor

- Chest pain. - Shortness of breath. - Fainting or severe dizziness. - A very slow heartbeat.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not take verapamil if you have: • Severe weakness of the heart’s left ventricle. • Very low blood pressure (systolic pressure below 90 mm Hg) or cardiogenic shock, when the heart cannot pump enough blood to the body. • Sick sinus syndrome or second- or third-degree heart block, unless you have a functioning artificial ventricular pacemaker. • Atrial flutter or atrial fibrillation together with an extra electrical pathway in the heart, such as Wolff-Parkinson-White or Lown-Ganong-Levine syndrome. • A known allergy to verapamil hydrochloride.

Interactions

Verapamil can interact with other medicines and prolong the intoxicating effects of alcohol. Important interactions to discuss with your doctor include: • Ivabradine: avoid combining it with verapamil because of the risk of an excessively slow heartbeat and problems with the heart’s electrical signals. • Beta-blockers, including timolol eye drops: the combination can slow the heartbeat, weaken the heart’s pumping action and cause heart block. Close monitoring is needed. • Clonidine: a severely slow heartbeat has been reported with this combination. Heart-rate monitoring is needed. • Other blood-pressure medicines, including prazosin: blood pressure may fall too low. • Digoxin and digitoxin: verapamil can increase their levels or slow their removal from the body. Your doctor may need to adjust treatment and monitor you closely, including when verapamil is stopped. • Simvastatin, lovastatin and atorvastatin: verapamil can increase exposure to these medicines, raising the risk of muscle injury, including serious muscle breakdown. Your prescriber may need to adjust treatment. • Heart-rhythm medicines such as disopyramide, flecainide and quinidine: these can affect heart function when used with verapamil. Disopyramide requires specific separation from verapamil, directed by your prescriber. Verapamil and quinidine should generally be avoided together in people with hypertrophic cardiomyopathy. • Erythromycin and ritonavir: these can increase verapamil levels. Rifampin and phenobarbital can lower verapamil exposure. Telithromycin has been associated with low blood pressure and abnormal, slow heart rhythms when combined with verapamil. • Lithium: harmful effects on the nervous system have been reported, even when lithium levels do not rise. Careful monitoring is needed. • Carbamazepine, cyclosporine and theophylline: verapamil may increase their levels. Increased carbamazepine levels may cause double vision, headache, poor coordination or dizziness. • Sirolimus, temsirolimus and everolimus: treatment adjustments may be needed because of interactions with verapamil. • Cimetidine: its effect on verapamil removal from the body is uncertain. • Inhaled anaesthetics and muscle-relaxing medicines used during procedures: verapamil may increase their effects on the heart or muscles. Discuss verapamil use with the team before a procedure. Alcohol may remain in the blood longer while taking verapamil, prolonging intoxication.
