About Jakafi
Overview
Jakafi (ruxolitinib) is a prescription kinase inhibitor supplied as tablets taken by mouth. It treats adults with intermediate- or high-risk myelofibrosis, including primary disease and disease following polycythemia vera or essential thrombocythemia, and adults with polycythemia vera when hydroxyurea has not worked well enough or cannot be tolerated. It also treats adults and children aged 12 years and older with acute graft-versus-host disease that has not responded to steroids, or chronic graft-versus-host disease after one or two courses of systemic treatment have failed. It works by blocking JAK1 and JAK2, enzymes that carry signals involved in blood cell production and immune function.
What it treats
- Intermediate- or high-risk myelofibrosis in adults, including primary myelofibrosis and myelofibrosis following polycythemia vera or essential thrombocythemia. - Polycythemia vera in adults who have not responded adequately to hydroxyurea or cannot tolerate it. - Acute graft-versus-host disease that has not responded to steroid treatment, in adults and children aged 12 years and older. - Chronic graft-versus-host disease after one or two lines of systemic treatment have failed, in adults and children aged 12 years and older.
Safety information
Side effects
Common side effects vary by the condition being treated. In myelofibrosis and polycythemia vera, they include low platelet counts, anemia (low red blood cell counts), bruising, dizziness, headache and diarrhea. In acute graft-versus-host disease, common effects include anemia, low platelet counts, low neutrophil counts (a type of white blood cell), infections and swelling. In chronic graft-versus-host disease, common effects include anemia, low platelet counts and infections, including viral infections. Contact your doctor promptly about signs of infection or concerns about low blood cell counts, which may require treatment changes or a transfusion. Serious infections should have resolved before starting Jakafi. Symptoms can worsen after treatment is interrupted or stopped; contact your doctor if this happens. Other serious risks listed in the label include major cardiovascular events, blood clots, non-melanoma skin cancer and other new cancers. Seek prompt medical attention for symptoms of a blood clot or a major cardiovascular event. Regular skin examinations and monitoring for new cancers are recommended, particularly for current or past smokers. Jakafi can also raise blood lipid levels; these should be checked 8 to 12 weeks after starting treatment and treated if needed.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
The FDA label lists no formal contraindications for Jakafi. However, serious infections should have resolved before treatment begins. Jakafi requires caution and monitoring for low platelet, red blood cell or neutrophil counts; infections; non-melanoma skin cancer; increased blood lipid levels; major cardiovascular events; blood clots; and additional cancers, particularly in current or former smokers. Periodic skin examinations are recommended, and blood lipid levels should be checked 8 to 12 weeks after starting treatment. Kidney or liver impairment may require a prescriber-directed dose adjustment or avoidance of treatment. Do not breastfeed during treatment. Symptoms may worsen if treatment is interrupted or stopped. Discuss any treatment changes with your prescriber.
Interactions
Tell your doctor if you take fluconazole or medicines that strongly block the enzyme CYP3A4. Fluconazole may require your doctor to adjust Jakafi treatment; some fluconazole doses must not be taken with Jakafi. Strong CYP3A4 inhibitors may require your doctor to reduce, temporarily interrupt or stop Jakafi. This adjustment recommendation does not apply to patients with acute or chronic graft-versus-host disease.

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
