Methotrexate tablets can cause embryo-fetal toxicity, including fetal death. For non-neoplastic diseases, methotrexate tablets are contraindicated in pregnancy. For neoplastic diseases, advise females and males of reproductive potential to use effective contraception.
Methotrexate tablets are contraindicated in patients with a history of severe hypersensitivity reactions to methotrexate, including anaphylaxis [Contraindications, Warnings and Precautions ].
Serious adverse reactions, including death, have been reported with methotrexate. Closely monitor for adverse reactions of the bone marrow, gastrointestinal tract, liver, lungs, skin, and kidneys. Withhold or discontinue methotrexate tablets as appropriate [Warnings and Precautions ].

About Methotrex
Overview
Methotrex contains methotrexate and requires a prescription. The supplied tablet label lists uses in adults with rheumatoid arthritis, severe psoriasis, mycosis fungoides, and non-Hodgkin lymphoma that has returned or not responded to treatment, with the lymphoma treatment given as part of a combination regimen. It also lists use in children with polyarticular juvenile idiopathic arthritis and in adults and children with acute lymphoblastic leukemia as part of combination maintenance chemotherapy. Methotrexate blocks an enzyme needed to make and repair DNA, interfering with cell multiplication; how it works in rheumatoid arthritis and psoriasis is unknown.
What it treats
- Acute lymphoblastic leukemia (ALL) in adults and children, as part of combination maintenance chemotherapy. - Mycosis fungoides in adults. - Non-Hodgkin lymphoma in adults that has returned or has not responded to treatment, as part of a metronomic combination regimen. - Rheumatoid arthritis in adults. - Polyarticular juvenile idiopathic arthritis (pJIA) in children. - Severe psoriasis in adults.
Safety information
Side effects
Common side effects include mouth sores, a low white blood cell count, nausea and abdominal discomfort. Serious side effects that require medical attention include allergic reactions, reduced production of blood cells, serious infections, and harmful effects on the digestive tract, liver, lungs, kidneys or nervous system. Skin reactions can also occur. Contact your doctor if you develop an infection or suspect a serious reaction. Monitoring for infections is needed during and after treatment. Methotrexate can also cause new cancers and tumour lysis syndrome, a potentially serious complication involving the breakdown of cancer cells. It can impair fertility, lower sperm counts and disrupt menstrual periods.

Before taking this medicine
Do not take methotrexate if you are pregnant and being treated for a non-cancer condition, or if you have previously had a severe allergic reaction to methotrexate, including anaphylaxis. Do not breastfeed during treatment. Monitoring is needed for infections during and after treatment and for harmful effects on the nervous system. Serious infections or nervous system toxicity may require treatment to be withheld or stopped by your prescriber. Other cancers can occur with methotrexate. Patients at risk of tumour lysis syndrome need preventive measures before treatment begins. Live vaccines are not recommended during treatment. Methotrexate can impair fertility, reduce sperm counts and disrupt menstrual periods.
