Increased risk of serious bacterial, fungal, viral and opportunistic infections leading to hospitalization or death, including tuberculosis (TB). Interrupt treatment with OLUMIANT if serious infection occurs until the infection is controlled. OLUMIANT should not be given to patients with active tuberculosis. Test for latent TB before and during therapy, except for COVID-19; treat latent TB prior to use. Monitor all patients for active TB during treatment, even patients with initial negative, latent TB test.
Higher rate of all-cause mortality, including sudden cardiovascular death with another Janus kinase inhibitor (JAK) vs. TNF blockers in rheumatoid arthritis (RA) patients.

About Olumiant
Overview
Olumiant (baricitinib) is a prescription medicine supplied as film-coated tablets. It treats adults with moderately to severely active rheumatoid arthritis after an inadequate response to one or more TNF blockers, and adults with severe alopecia areata. It also treats COVID-19 in hospitalized adults who need supplemental oxygen, mechanical ventilation or extracorporeal membrane oxygenation (ECMO). It works by blocking Janus kinase (JAK) enzymes that carry signals involved in immune cell function and blood cell production.
What it treats
- Moderately to severely active rheumatoid arthritis in adults who have had an inadequate response to one or more TNF blockers. - COVID-19 in hospitalized adults requiring supplemental oxygen, non-invasive or invasive mechanical ventilation, or extracorporeal membrane oxygenation (ECMO). - Severe alopecia areata in adults.
Safety information
Side effects
Reported side effects vary by the condition being treated. Commonly reported effects include upper respiratory tract infections, nausea, headache, acne, tiredness, abdominal pain, weight gain, cold sores and shingles. Other reported effects include urinary tract and lower respiratory tract infections, infections around hair follicles, and genital yeast infections. Blood tests may show increased cholesterol or other blood fats, liver enzymes, creatine phosphokinase or platelet counts, or reduced red blood cells or certain white blood cells. Your doctor may monitor for these changes. Serious risks include serious infections, cancer and lymphoproliferative disorders, major cardiovascular events, blood clots in deep veins or the lungs, serious allergic reactions, and holes in the stomach or intestinal wall. The label also warns of a risk of death. Seek urgent medical attention if you suspect a serious reaction. Stop taking Olumiant if a serious allergic reaction occurs. Contact a doctor promptly about new abdominal symptoms. Low blood sugar can occur in people with diabetes. If you have diabetes, tell your healthcare provider if you develop symptoms of low blood sugar; closer blood glucose monitoring may be needed.

When to call your doctor

Before taking this medicine
Olumiant is not recommended for people with rheumatoid arthritis or alopecia areata who have severe liver impairment or kidney function measured by an eGFR below 30 mL/min/1.73 m². For people with COVID-19, it is not recommended if eGFR is below 15 mL/min/1.73 m², or if they are on dialysis, have end-stage kidney disease or have acute kidney injury. It has not been studied in people with COVID-19 and severe liver impairment. Serious allergic reactions have been reported. Stop Olumiant if a serious allergic reaction occurs. People at increased risk of gastrointestinal perforation, a hole in the stomach or bowel, require monitoring, and new abdominal symptoms need prompt assessment. People with diabetes may need more frequent blood glucose checks and should tell their healthcare provider if symptoms of low blood sugar develop. Blood tests are needed to monitor lymphocytes, neutrophils, haemoglobin, liver enzymes and blood lipids. Avoid live vaccines during treatment. Based on animal studies, Olumiant may harm an unborn baby. Do not breastfeed during treatment.
Interactions
Tell your doctor if you take probenecid or another strong organic anion transporter 3 (OAT3) inhibitor. These medicines require your prescriber to reduce your Olumiant dose.

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
