Overview
Diclofenac sodium topical gel is a nonsteroidal anti-inflammatory drug (NSAID) for use on the skin. It is used to treat actinic keratoses (AK). How it works to treat actinic keratoses is not known.

Safety information
Side effects
The most common side effects of diclofenac sodium gel are skin reactions where it is applied, including itching, rash, dry skin, scaling and contact dermatitis (skin inflammation). Pain, tingling and swelling at the application site have also been reported. Burning and allergic reactions have been reported after approval, but their frequency is unknown. Stop using the gel and contact a doctor immediately if a rash or other sign of an allergic reaction develops. Serious skin reactions, including blistering and peeling, can be life-threatening and may occur without warning. Seek emergency help for a severe allergic reaction, heart attack, stroke or bleeding from the stomach or intestines. Digestive tract ulcers, bleeding and perforation can occur at any time, sometimes without warning, and can be fatal. Other serious risks include liver injury, new or worsening high blood pressure, heart failure, fluid retention, kidney injury, high blood potassium and blood disorders, including anemia. Stop using the gel and seek prompt medical assessment for possible liver symptoms, such as yellowing of the skin or eyes, dark urine, unusual tiredness or upper-right abdominal tenderness. Contact a doctor promptly for worsening asthma or swelling.

Before taking this medicine
Do not use diclofenac sodium topical gel if you are allergic to diclofenac or any ingredient, have had a serious skin reaction to an NSAID, or have developed asthma, hives or another allergic reaction after taking aspirin or another NSAID. Do not use it in connection with coronary artery bypass graft (CABG) surgery. Do not apply it to damaged skin, including eczema, infected areas, burns or open wounds. Tell your prescriber if you have asthma, heart disease, a recent heart attack, high blood pressure, heart failure, kidney or liver disease, dehydration, anemia, a bleeding disorder, or a history of stomach ulcers or digestive tract bleeding. Use should generally be avoided after a recent heart attack, in severe heart failure or in advanced kidney disease. NSAIDs can cause heart attack, stroke, and serious digestive tract bleeding, ulcers or perforation, sometimes without warning. Older adults and people in poor general health have a greater risk of serious digestive tract problems. Blood pressure, kidney function, liver function or blood counts may need monitoring. Avoid using the gel with oral NSAIDs. Tell your prescriber about aspirin, blood thinners, oral corticosteroids, SSRI antidepressants, blood pressure medicines, diuretics and medicines that may affect the liver. Alcohol use and smoking also increase the risk of digestive tract bleeding. Use during pregnancy should be limited between about 20 and 30 weeks and avoided from about 30 weeks onward because of risks to the unborn baby's kidneys and circulation. NSAIDs may cause reversible difficulty conceiving; tell your prescriber if you are having trouble becoming pregnant. Stop using the gel at the first rash or other sign of an allergic reaction. Seek emergency help for a severe allergic reaction. Stop use and seek medical advice if signs of liver injury develop, such as yellowing skin or eyes or dark urine. Avoid sunlight or artificial UV exposure on treated areas, and keep the gel away from eyes and mucous membranes.
Interactions
Diclofenac sodium topical gel can interact with the following medicines: • Blood thinners such as warfarin, antiplatelet medicines such as aspirin, and SSRI or SNRI antidepressants: using these together increases bleeding risk. Your doctor should monitor for signs of bleeding. • Aspirin used for pain relief: combined use is generally not recommended because of increased bleeding risk. Diclofenac gel does not replace aspirin prescribed for cardiovascular protection. • Other nonsteroidal anti-inflammatory drugs (NSAIDs) or salicylates, such as diflunisal and salsalate: combined use is not recommended because it increases the risk of stomach and intestinal harm. • ACE inhibitors, angiotensin receptor blockers (ARBs), and beta-blockers: diclofenac may reduce their blood-pressure-lowering effect. ACE inhibitors and ARBs may also increase the risk of worsening kidney function when combined with diclofenac, particularly in older people, people with reduced fluid levels, or those with kidney problems. Blood pressure and kidney monitoring may be needed. • Diuretics, including furosemide and thiazides: diclofenac may reduce their effect. Your doctor should monitor kidney function and how well the diuretic is working. • Digoxin and lithium: diclofenac may increase their levels in the blood. Digoxin blood tests or monitoring for lithium toxicity may be needed. • Methotrexate: combined use may increase the risk of harmful effects, including low blood cell counts and kidney problems. Monitoring is needed. • Cyclosporine: combined use may increase kidney damage risk. Kidney monitoring is needed. • Pemetrexed: combined use may increase harmful effects on blood cell production, the kidneys, and the digestive tract. Diclofenac should be avoided around pemetrexed treatment; your prescriber should manage the timing. Additional monitoring is important for people with reduced kidney function.
