About Xgeva
Overview
Denosumab is a monoclonal antibody medicine used to help prevent bone complications in people with multiple myeloma or solid tumors that have spread to bone. It also treats giant cell tumor of bone in adults and adolescents whose bones have finished growing, when surgery is not possible or would likely cause serious harm. It is also used for high blood calcium caused by cancer that has not responded to bisphosphonate treatment. It works by blocking RANKL, a protein that activates the cells responsible for breaking down bone and releasing calcium.
What it treats
- Prevention of skeletal-related complications in patients with multiple myeloma or solid tumors that have spread to bone. - Treatment of giant cell tumor of bone in adults and adolescents whose bones have finished growing, when the tumor cannot be removed surgically or surgery is likely to cause serious harm. - Treatment of high blood calcium caused by cancer that has not responded to bisphosphonate treatment.
Safety information
Side effects
Common side effects vary by the condition being treated: For bone metastases from solid tumors, these include tiredness or weakness, low blood phosphate and nausea. For multiple myeloma, these include diarrhea, nausea, anemia, back pain, low platelet counts, swelling of the hands or feet, low blood calcium, upper respiratory tract infection, rash and headache. For giant cell tumor of bone, these include joint pain, headache, nausea, back pain, tiredness and pain in the arms or legs. For high blood calcium caused by cancer, these include nausea, shortness of breath, reduced appetite, headache, swelling of the hands or feet, vomiting, anemia, constipation and diarrhea. Serious side effects include severe allergic reactions, including anaphylaxis, and severely low blood calcium, which can cause symptoms and has been fatal in some cases. Seek immediate medical help for a suspected severe allergic reaction, and contact your doctor promptly about suspected low blood calcium. Calcium levels need monitoring during treatment, especially in the first weeks, and calcium and vitamin D supplementation is required. Other serious risks include damage to the jawbone and unusual thighbone fractures. Contact your doctor about jaw symptoms or thigh or groin pain. An oral examination is needed before treatment, and invasive dental procedures should be avoided during treatment. After treatment stops, high blood calcium can occur in people with giant cell tumor of bone or growing skeletons. Multiple spinal fractures can also occur after stopping treatment. Discuss stopping treatment with your doctor so these risks can be assessed and monitored.

Before taking this medicine
Do not use Xgeva if you have low blood calcium or a known clinically significant allergy to denosumab products. Low calcium must be corrected before treatment. Do not receive other denosumab medicines at the same time. Denosumab can cause severe low blood calcium, including fatal cases. Calcium levels should be monitored, especially during the first weeks of treatment, and calcium and vitamin D supplements are needed. People with severe kidney impairment or receiving dialysis are at particular risk. Serious allergic reactions, including anaphylaxis, can occur. Treatment must be permanently stopped if a clinically significant allergic reaction occurs. Jawbone damage has been reported. An oral examination is needed before treatment, and invasive dental procedures should be avoided during treatment. Report thigh or groin pain so that a possible unusual thighbone fracture can be assessed. After treatment stops, high blood calcium can occur in people with giant cell tumor of bone or growing skeletons. Monitoring is needed in these groups. The risk of multiple spinal fractures should also be assessed when treatment is stopped. Denosumab can harm an unborn baby. People who could become pregnant should use effective contraception. Use in children is recommended only for adolescents whose skeletons have finished growing and who have giant cell tumor of bone.
