About Alimta
Overview
Alimta (pemetrexed) is a prescription cancer medicine supplied in a vial for preparation as an intravenous infusion. It is used alone or with other cancer medicines for certain types of advanced or metastatic non-squamous non-small cell lung cancer; it is not indicated for squamous cell lung cancer. It is also used with cisplatin as an initial treatment for malignant pleural mesothelioma when surgery cannot remove the cancer or surgery aimed at curing it is not an option. It works by blocking folate-dependent processes that cells need to make genetic material and multiply.
What it treats
• Non-squamous non-small cell lung cancer (NSCLC): initial treatment of metastatic disease without EGFR or ALK gene changes, with pembrolizumab and platinum chemotherapy; initial treatment of locally advanced or metastatic disease with cisplatin; maintenance treatment alone when disease has not progressed after four cycles of initial platinum-based chemotherapy; or treatment alone for recurrent metastatic disease after prior chemotherapy. Not indicated for squamous cell NSCLC. • Malignant pleural mesothelioma: initial treatment with cisplatin when the cancer cannot be surgically removed or the patient is not a candidate for surgery intended to cure it.
Safety information
Side effects
Common side effects depend on whether Alimta is used alone or with other medicines. When used alone, the most common are tiredness, nausea and loss of appetite. With cisplatin, common side effects include vomiting, low white blood cell counts, anemia, inflammation of the mouth or throat, low platelet counts and constipation. With pembrolizumab and platinum chemotherapy, common side effects include tiredness or weakness, nausea, constipation, diarrhea, decreased appetite, rash, vomiting, cough, shortness of breath and fever. Contact a doctor immediately for severe blistering or peeling skin, or new or worsening unexplained breathing symptoms. Serious risks include severe bone marrow suppression, which lowers blood cell counts and increases the risk of infection; severe and sometimes fatal kidney failure; life-threatening skin reactions; and lung inflammation. Alimta can also trigger a reaction in areas previously treated with radiation, even weeks to years later. Alimta can harm an unborn baby. Use effective contraception and contact your doctor if you become pregnant.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
Do not use Alimta if you have had a severe allergic reaction to pemetrexed. It must not be given if your neutrophil count is below 1,500 cells/mm³, your platelet count is below 100,000 cells/mm³, or your creatinine clearance is below 45 mL/min. Alimta can cause severe bone marrow suppression, increasing the risk of infection, and severe, sometimes fatal, kidney failure. Oral folic acid and vitamin B12 injections are required to reduce blood-related and gastrointestinal toxicity. Severe blistering or peeling skin reactions require permanent discontinuation. New or worsening unexplained lung symptoms require treatment to be withheld; confirmed lung inflammation requires permanent discontinuation. Previous radiation treatment can lead to a radiation recall reaction, even weeks to years later, which also requires permanent discontinuation. Alimta can harm an unborn baby. Use effective contraception. Do not breastfeed during treatment.
Interactions
Ibuprofen may increase the risk of toxicity from Alimta in patients with mild to moderate kidney impairment. Tell your doctor if you take ibuprofen, as they may need to adjust its use.

Storage
Store in the original carton below 25°C, protected from light, unless the pack states refrigeration. Do not freeze. Keep all medicines out of the sight and reach of children.
