About Dilantin

Overview

Dilantin is a prescription seizure medicine containing phenytoin sodium in extended-release capsules. It is used to treat tonic-clonic (grand mal) and psychomotor (temporal lobe) seizures, and to prevent and treat seizures during or after brain surgery. Its exact action is not fully understood, but it is thought to block sodium channels in nerve cells, reducing repeated rapid electrical signals that can cause seizures.

Side effects

The most common side effects of Dilantin affect the nervous system and include involuntary eye movements, unsteadiness, slurred speech, reduced coordination, drowsiness and confusion. Other reported effects include dizziness, headache, trouble sleeping, nausea, vomiting, constipation, gum overgrowth and changes in taste. Contact a doctor immediately if you develop a rash, even without fever. Serious skin reactions, including Stevens–Johnson syndrome and toxic epidermal necrolysis, can be fatal. The label advises stopping Dilantin at the first sign of a rash unless it is clearly unrelated to the medicine. Seek emergency medical help for swelling of the face, around the mouth or in the upper airway, or a severe allergic reaction. Serious allergic reactions can involve several organs. Slow heartbeat and cardiac arrest have also been reported. Contact a doctor promptly for new or worsening depression, suicidal thoughts or behaviour, or unusual changes in mood or behaviour. Other serious reported effects include liver injury or acute liver failure, blood cell and bone marrow disorders that can be fatal, effects on vitamin D and bones, and high blood sugar. Long-term use has been associated with nerve damage affecting sensation; shrinkage of the cerebellum, a part of the brain involved in coordination, has also been reported. Abrupt withdrawal can trigger prolonged seizures. Any planned reduction or discontinuation should be managed by the prescriber.

When to call your doctor

- Seek immediate medical attention for a new rash, especially with fever, blisters or peeling skin. - Get help immediately for swelling of the face, around the mouth or in the throat. - Contact a doctor immediately for suicidal thoughts or behavior, worsening depression, or unusual mood or behavior changes. - Seek immediate medical attention for a slow heartbeat. - Get emergency help for a seizure that does not stop.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not take Dilantin if you have had an allergic reaction to phenytoin, any of its ingredients or other hydantoin medicines, or if phenytoin has previously caused acute liver injury. Do not take it with delavirdine, as this combination may reduce delavirdine’s effectiveness and lead to resistance. Special caution is needed during pregnancy because exposure to phenytoin before birth may increase the risk of birth defects and other developmental problems. People with kidney or liver impairment, or low blood albumin, need monitoring of unbound phenytoin levels. Dilantin can cause serious skin reactions, severe allergic reactions involving multiple organs, liver injury, blood disorders, a slow heartbeat and cardiac arrest. Seek immediate medical assessment for a rash or signs of an allergic reaction. Stop taking it and seek immediate help if swelling of the face, around the mouth or upper airway occurs. Watch for new or worsening depression, suicidal thoughts or unusual changes in mood or behaviour. Do not stop Dilantin suddenly, as withdrawal can trigger prolonged seizures. Any planned reduction or discontinuation should be managed by your prescriber.

Interactions

Phenytoin can interact with other medicines. Some medicines can significantly increase phenytoin levels in the blood, raising the risk of toxicity. Phenytoin can also affect how the liver processes other medicines. Discuss other medicines you take with your doctor. If an interaction is suspected, your doctor may check your phenytoin blood level.

Storage
