FDA boxed warning(a) premature discontinuation of eliquis increases the risk of thrombotic events
(B) SPINAL/EPIDURAL HEMATOMA
(A) PREMATURE DISCONTINUATION OF ELIQUIS INCREASES THE RISK OF THROMBOTIC EVENTS
Premature discontinuation of any oral anticoagulant, including ELIQUIS, increases the risk of thrombotic events. If anticoagulation with ELIQUIS is discontinued for a reason other than pathological bleeding or completion of a course of therapy, consider coverage with another anticoagulant
Epidural or spinal hematomas may occur in patients treated with ELIQUIS who are receiving neuraxial anesthesia or undergoing spinal puncture. These hematomas may result in long-term or permanent paralysis. Consider these risks when scheduling patients for spinal procedures. Factors that can increase the risk of developing epidural or spinal hematomas in these patients include:
use of indwelling epidural catheters
concomitant use of other drugs that affect hemostasis, such as nonsteroidal anti-inflammatory drugs (NSAIDs), platelet inhibitors, other anticoagulants
a history of traumatic or repeated epidural or spinal punctures
a history of spinal deformity or spinal surgery
optimal timing between the administration of ELIQUIS and neuraxial procedures is not known
Monitor patients frequently for signs and symptoms of neurological impairment. If neurological compromise is noted, urgent treatment is necessary
Consider the benefits and risks before neuraxial intervention in patients anticoagulated or to be anticoagulated
About Eliquis
Overview
Eliquis is a prescription medicine containing apixaban, used to reduce the risk of stroke and blood clots travelling through the body in adults with nonvalvular atrial fibrillation. It also treats deep vein thrombosis (DVT) and pulmonary embolism (PE) in adults and reduces the risk of these clots returning after initial treatment. It is also used to help prevent DVT, which can lead to PE, in adults after hip or knee replacement surgery. Apixaban blocks factor Xa, a protein involved in blood clotting, helping reduce clot formation.
What it treats
- Reducing the risk of stroke and blood clots travelling elsewhere in the body in adults with nonvalvular atrial fibrillation. - Preventing deep vein thrombosis (DVT), which may lead to pulmonary embolism (PE), in adults after hip or knee replacement surgery. - Treating DVT and PE in adults, and reducing the risk of recurrence after initial treatment. - Treating venous thromboembolism (VTE) and reducing the risk of recurrence in children from birth, after at least 5 days of initial anticoagulant treatment.
Safety information
Side effects
The most common side effects in adults are related to bleeding. In children, the most common side effects are headache, vomiting and excessive menstrual bleeding. Eliquis can cause serious, potentially fatal bleeding. Report any signs or symptoms of blood loss to a doctor immediately or go to an emergency room. Taking Eliquis with aspirin, other medicines that prevent blood clots, NSAIDs or certain antidepressants can increase the risk of bleeding. Stopping Eliquis early without adequate alternative treatment to prevent blood clots increases the risk of blood clots, including stroke. Speak to your prescriber before stopping treatment.

A boxed warning is the strongest warning the US Food and Drug Administration requires on a medicine label. Text is taken from the official label, with internal section references removed.
When to call your doctor
If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
Do not take Eliquis if you have active abnormal bleeding or have had a severe allergic reaction to Eliquis, such as anaphylaxis. Eliquis is not recommended during pregnancy or for people with an artificial heart valve, triple-positive antiphospholipid syndrome, or severe liver impairment. Do not breastfeed while taking Eliquis. Eliquis can cause serious, potentially fatal bleeding. Bleeding risk increases when it is taken with aspirin, other antiplatelet medicines, other anticoagulants, heparin, clot-dissolving medicines, SSRI or SNRI antidepressants, or nonsteroidal anti-inflammatory drugs (NSAIDs). Report signs of bleeding or blood loss immediately, or go to an emergency department. Do not stop Eliquis without medical advice. Stopping treatment early without adequate alternative anticoagulation increases the risk of blood clots. Your prescriber may need to arrange another anticoagulant.
Interactions
Ketoconazole, itraconazole and ritonavir can raise apixaban levels and increase bleeding risk. Your prescriber may need to adjust your treatment or avoid these combinations. Their use with Eliquis has not been studied in children. Rifampin, carbamazepine, phenytoin and the herbal supplement St. John’s wort can lower apixaban levels, increasing the risk of stroke and other blood clots. These combinations should be avoided. Aspirin, other antiplatelet medicines such as clopidogrel, blood thinners such as heparin, clot-dissolving medicines and regular use of nonsteroidal anti-inflammatory drugs (NSAIDs) can increase bleeding risk with Eliquis. Tell your doctor if you take any of these medicines or supplements.
