(A) REDUCED EFFICACY IN NONVALVULAR ATRIAL FIBRILLATION PATIENTS WITH CRCL > 95 ML/MIN: SAVAYSA should not be used in patients with CrCL > 95 mL/min. In the ENGAGE AF-TIMI 48 study, nonvalvular atrial fibrillation patients with CrCL > 95 mL/min had an increased rate of ischemic stroke with SAVAYSA 60 mg once daily compared to patients treated with warfarin. In these patients another anticoagulant should be used.
(B) PREMATURE DISCONTINUATION OF SAVAYSA INCREASES THE RISK OF ISCHEMIC EVENTS: Premature discontinuation of any oral anticoagulant in the absence of adequate alternative anticoagulation increases the risk of ischemic events. If SAVAYSA is discontinued for a reason other than pathological bleeding or completion of a course of therapy, consider coverage with another anticoagulant as described in the transition guidance.
(C) SPINAL/EPIDURAL HEMATOMA: Epidural or spinal hematomas may occur in patients treated with SAVAYSA who are receiving neuraxial anesthesia or undergoing spinal puncture. These hematomas may result in long-term or permanent paralysis. Consider these risks when scheduling patients for spinal procedures.
About Savaysa
Overview
Savaysa (edoxaban) is a prescription blood thinner used to reduce the risk of stroke and blood clots elsewhere in the body in people with nonvalvular atrial fibrillation. For this use, it should not be used when creatinine clearance, a measure of kidney function, is above 95 mL/min because of a higher risk of ischemic stroke compared with warfarin. It also treats deep vein thrombosis and pulmonary embolism after initial treatment with an injected anticoagulant. It works by blocking factor Xa, a protein involved in blood clotting, to reduce clot formation.
What it treats
- Reducing the risk of stroke and systemic embolism (blood clots travelling elsewhere in the body) in people with nonvalvular atrial fibrillation. For this use, Savaysa should not be used when creatinine clearance is above 95 mL/min because of a higher risk of ischemic stroke compared with warfarin. - Treating deep vein thrombosis (DVT) and pulmonary embolism (PE) after initial treatment with an injectable anticoagulant.
Safety information
Side effects
Common side effects include bleeding and anemia (a low red blood cell count). In people treated for deep vein thrombosis (DVT) or pulmonary embolism (PE), rash and abnormal liver function tests are also common. Bleeding can be serious or fatal. Contact a doctor promptly if you have signs or symptoms of bleeding or blood loss. Stopping Savaysa increases the risk of stroke in people with nonvalvular atrial fibrillation. Discuss stopping treatment with your doctor before making any changes.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
Do not take Savaysa if you have active abnormal bleeding. It can cause serious and potentially fatal bleeding. Signs or symptoms of blood loss need prompt medical assessment. Savaysa is not recommended for people with mechanical heart valves, moderate to severe mitral stenosis, moderate or severe liver impairment, or triple-positive antiphospholipid syndrome, which is associated with an increased risk of blood clots. Tell your prescriber if you have impaired kidney function, as this may require a dose adjustment. Do not breastfeed while taking Savaysa.
Interactions
Tell your doctor if you take other anticoagulants (blood thinners), antiplatelet medicines, thrombolytics (clot-dissolving medicines), or SSRI or SNRI antidepressants. The label advises avoiding these medicines with Savaysa because they increase the risk of bleeding. Rifampin should also be avoided with Savaysa.
