About Ofev

Overview

Ofev contains nintedanib, a medicine called a kinase inhibitor. It is used in adults to treat idiopathic pulmonary fibrosis (IPF) and chronic fibrosing interstitial lung diseases (ILDs) that continue to worsen. It is also used to slow the decline in lung function in adults with systemic sclerosis-associated interstitial lung disease (SSc-ILD). It works by blocking certain enzyme signals involved in the development of scar tissue in the lungs.

What it treats

- Idiopathic pulmonary fibrosis (IPF) in adults. - Chronic fibrosing interstitial lung diseases (ILDs) that progressively worsen in adults. - Systemic sclerosis-associated interstitial lung disease (SSc-ILD) in adults, to slow the decline in lung function.

Safety information

Side effects

Common side effects of Ofev include diarrhea, nausea, abdominal pain, vomiting, increased liver enzymes, reduced appetite, headache, weight loss and high blood pressure. Serious risks include liver injury, which can be severe or fatal; blood clots in arteries; bleeding; a hole in the stomach or intestine (gastrointestinal perforation); and high levels of protein in the urine. Contact your doctor if you develop severe or persistent diarrhea, nausea or vomiting, bleeding, or new or worsening protein in the urine. Liver injury occurs most often within the first three months of treatment. Liver blood tests are needed before treatment, regularly during the first three months and periodically afterward. Ofev can harm an unborn baby. Women who could become pregnant should use highly effective contraception. Vomiting or diarrhea may reduce the effectiveness of oral hormonal contraceptives, so an alternative highly effective method is needed in these circumstances.

- Seek medical attention immediately for severe diarrhea, nausea or vomiting that continues despite treatment. - Seek medical attention immediately for unusual bleeding.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

The FDA label lists no formal contraindications for Ofev. However, it is not recommended for people with moderate or severe liver impairment. Mild liver impairment requires additional care. Ofev can harm an unborn baby. Women who could become pregnant should use highly effective contraception. Vomiting or diarrhea may make oral hormonal contraceptives less reliable; alternative highly effective contraception is advised in these circumstances. Breastfeeding is not recommended during treatment. Conditions requiring caution include: - Liver problems: serious liver injury, including fatal cases, has been reported. Liver blood tests are needed before treatment, regularly during the first three months and periodically afterward. - Higher cardiovascular risk, including known coronary artery disease: blood clots in arteries have been reported. - A known bleeding risk: Ofev should be used only if the expected benefit outweighs the risk. - Recent abdominal surgery, a history of diverticular disease, or use of corticosteroids or NSAIDs: these require caution because a hole in the stomach or bowel wall has been reported. In people at known risk, treatment should be used only if the expected benefit outweighs that risk. - Severe kidney impairment or end-stage kidney disease: safety and effectiveness have not been studied. Diarrhea, nausea or vomiting require prompt management; treatment must be stopped if severe symptoms persist despite treatment for them. New or worsening protein in the urine may require a treatment interruption. Smoking reduces exposure to Ofev and may affect its effectiveness.

Interactions

Medicines that inhibit P-gp and CYP3A4 may increase the amount of nintedanib in your body. Tell your doctor about other medicines you take. If used together, your doctor should monitor you closely for how well you tolerate Ofev.

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
