About Zeposia
Overview
Zeposia is a prescription medicine containing ozanimod, supplied as capsules taken by mouth. It treats relapsing forms of multiple sclerosis in adults, including clinically isolated syndrome, relapsing-remitting disease and active secondary progressive disease. It also treats moderately to severely active ulcerative colitis in adults. It keeps certain immune cells in the lymph nodes, which may reduce their movement into the brain, spinal cord and intestine, although its exact treatment mechanism is not known.
What it treats
- Relapsing forms of multiple sclerosis (MS) in adults, including clinically isolated syndrome, relapsing-remitting disease and active secondary progressive disease. - Moderately to severely active ulcerative colitis (UC) in adults.
Safety information
Side effects
Common side effects in people treated for multiple sclerosis include upper respiratory infections, raised liver enzymes, a drop in blood pressure when standing, urinary tract infections, back pain and high blood pressure. In people treated for ulcerative colitis, common side effects include raised liver test results, upper respiratory infections and headache. Serious side effects that warrant contacting a doctor include infections; progressive multifocal leukoencephalopathy (PML), a serious brain infection; a slow heart rate or problems with the heart’s electrical conduction; liver injury; increased blood pressure; reduced lung function; macular edema (swelling in the part of the eye responsible for central vision); and skin cancers. Contact your doctor about changes in vision or suspicious skin lesions. Diabetes and uveitis increase the risk of macular edema. Other serious risks listed in the label include posterior reversible encephalopathy syndrome (PRES), additional suppression of the immune system from previous immune-suppressing or immune-modifying treatments, severe worsening of multiple sclerosis disability after stopping Zeposia, and immune system effects that continue after treatment ends. Monitoring for infection should continue for 3 months after stopping. Zeposia may harm an unborn baby. Women who could become pregnant should use effective contraception during treatment and for 3 months after stopping.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
Do not take Zeposia if you: • Have had a heart attack, unstable angina, stroke, transient ischemic attack (TIA), worsening heart failure requiring hospital care, or Class III or IV heart failure within the last 6 months. • Have certain heart rhythm or electrical conduction disorders, Mobitz type II second-degree or third-degree AV block, sick sinus syndrome, or sino-atrial block, unless you have a functioning pacemaker. • Have severe untreated sleep apnea. • Are taking a monoamine oxidase (MAO) inhibitor. Do not start Zeposia while you have an active infection. It may increase infection risk; a complete blood count is needed before treatment, with monitoring for infections during treatment and for 3 months after stopping. Treatment should be withheld at the first sign or symptom suggesting progressive multifocal leukoencephalopathy (PML). Zeposia may temporarily slow the heart rate. An ECG is needed before treatment, and a cardiology consultation may be needed if you have heart conduction problems or take other medicines that slow the heart rate. Use is not recommended in severe liver impairment. Liver enzymes should be checked before and periodically during treatment. Zeposia should be stopped if liver injury occurs without another cause. Zeposia may harm an unborn baby. Women who could become pregnant should use effective contraception during treatment and for 3 months after stopping. Blood pressure should be monitored during treatment. Zeposia may reduce lung function, which should be assessed when clinically needed. It also increases the risk of macular edema, with greater risk in people with diabetes or uveitis. Eye examinations are needed near the start of treatment, periodically during treatment, and whenever vision changes occur. Skin examinations are recommended before or shortly after starting treatment and periodically thereafter; suspicious skin lesions should be evaluated.
Interactions
Zeposia can interact with the following medicines and vaccines: - MAO inhibitors, including selegiline, phenelzine and linezolid, must not be used with Zeposia. At least 14 days must pass after stopping Zeposia before starting an MAO inhibitor. - Gemfibrozil and other strong CYP2C8 inhibitors can increase levels of ozanimod’s active breakdown products and may increase side effects. Use together is not recommended. - Rifampin and other strong CYP2C8 inducers can lower levels of ozanimod’s active breakdown products and may reduce its effectiveness. Use together should be avoided. - Cancer treatments and medicines that alter or suppress the immune system may have additional immune effects when used with Zeposia or when switching treatments. Starting Zeposia after alemtuzumab is not recommended. - Heart rhythm medicines such as quinidine, procainamide, amiodarone and sotalol require advice from a cardiologist before starting Zeposia. Medicines that prolong the heart’s QT interval also need cardiologist review; Zeposia should generally not be started alongside those known to cause abnormal heart rhythms. - Taking both a beta blocker and a heart-rate-lowering calcium channel blocker, such as verapamil or diltiazem, may further slow the heart rate. Zeposia should generally not be started with this combination unless a cardiologist has been consulted. - Vaccines may be less effective during treatment and for up to three months after stopping Zeposia. Avoid live attenuated vaccines during this period because they may cause infection.

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
